摘要
目的 探讨母体和脐血血清胰岛素样生长因子 1(IGF 1)、C肽 (CP)及胰岛素样生长因子结合蛋白 1(IGFBP 1)质量浓度对新生儿出生体重的影响。方法 选择 12例大于胎龄儿 (LGA)、15例适于胎龄儿 (AGA)和 2 0例小于胎龄儿 (SGA) ,分为 3组 ,分别抽取脐血和其母外周静脉血 ,分离血清。采用ELISA法测定IGF 1、CP和IGFBP 1。结果 (1)LGA组脐血IGF 1、CP、IGFBP 1质量浓度 (ng·mL-1)分别为 5 9 76± 14 5 0 ,1 36± 0 5 8,5 3 78±19 6 7;AGA组为 33 6 5± 12 74 ,0 90± 0 4 5 ,10 4 79± 2 7 6 0 ;SGA组为 17 0 5± 8 10 ,0 5 2± 0 2 2 ,16 9 2 1± 5 5 30 ;3组间差异有显著性意义 (P <0 0 1)。 (2 )LGA组新生儿母亲血清IGF 1、CP、IGFBP 1质量浓度 (ng·mL-1)分别为2 6 7 15± 99 4 3,2 84± 0 93,15 8 5 0± 5 1 2 5 ;AGA组母亲分别为 2 37 34± 10 5 5 6 ,2 71± 0 72 ,170 78± 6 3 6 9;SGA组分别为 2 2 9 5 3± 94 17,2 5 9± 0 83,188 90± 74 0 4 ;3组间比较差异均无显著性。但同组患儿母血IGF 1、CP及IGFBP 1均显著高于脐血水平 (除SGA组母血与脐血IGFBP 1差异无显著性外 )。 (3)新生儿出生体重与脐血IGF 1、CP呈正相关 (r分别为 0 814、0 6 5 4 ,P <0 0 1) ;与IGFBP 1?
Objective To explore the relationship between birth weight and maternal and fetal serum levels of insulin like growth factor 1 (IGF 1),C peptide (CP) and insulin like growth factor binding protein 1 (IGFBP 1).Methods Samples of maternal blood and matched umbilical cord blood were collected at the time of delivery from 12 pairs of mother and newborn with large gestational age (LGA group),15 pairs with appropriate gestational age (AGA group),20 pairs with small gestational age (SGA group).Serum IGF 1,CP and IGFBP 1 were measured by ELISA.Results (1) Cord serum IGF 1,CP and IGFBP 1(ng/mL) were 59 76±14 50,1 36±0 58 and 53 78±19 67 respectively in LGA group,33 65±12 74,0 90±0 45,104 79±27 60 in AGA group and 17 05±8 10,0 52±0 22 and 169 21±55 30 in SGA group.There were significant differences among three groups.(2) Maternal blood IGF 1,CP and IGFBP 1 levels(ng/mL) were 267 15±99 43,2 84±0 93 and 158 50±51 25 respectively in LGA group,237 34±105 56,2 71±0 72 and 170 78±63 69 in AGA group,and 229 53±94 17,2 59±0 83 and 188 90±74 04 in SGA group.There were no significant differences among three groups.But in the same group,IGF 1,CP and IGFBP 1 levels were markedly higher in maternal blood than in matched cord serum except IGFBP 1 in SGA group.(3) Birth weight was positively correlated with IGF 1 and CP levels and negatively with IGFBP 1 in cord blood.But it had no correlation with maternal IGF 1 ,IGFBP 1 and CP levels.Conclusion It is fetal blood IGF 1,CP and IGFBP 1 that have close relationship with fetal growth.So nutrition insulin IGF axis may play an important role in regulation of intrauterine growth.
