超高度近视有晶状体眼前房负镜植入两年临床观察 被引量:1

摘要 目的 研究前房负镜植入术矫正超高度近视的临床效果。方法 高度近视患者 11例 ( 2 0眼 )。屈光度数最高 -2 9D ,最低 -9 45D。术前进行眼轴长度、前房深度、角膜直径、角膜内皮计数、裸眼视力、矫正视力检查。计算人工晶状体度数。显微手术植入前房型人工晶状体。术后随访 2年 ,观察裸眼视力、最佳矫正视力、眼压、角膜内皮计数、前房反应、人工晶状体位置、瞳孔改变等。结果 术后 2年所有患者平均裸眼视力 0 73 ,矫正视力 0 92。平均眼压为14 17mmHg ,无一例眼压升高。角膜内皮细胞计数 ( 2 499 3± 3 19 6)个 /mm2 ,平均损失率 3 4% ,与术前无明显差异。ICL袢脚均未造成对房角的影响。所有患者瞳孔对光反应良好 ,无一例出现白内障。结论 前房负镜植入矫正高度近视 ,尤其是高于 -12D的超高度近视 ,2年观察临床效果令人满意。 ObjectiveTo investigate the long-term effect of phackic anterior chamber intraocular lens implantation for correcting severe myopia.MethodsEleven patients(20 eyes)with -9.45 D—-29 D myopia were selected.Ocular axial length,corneal diameter,corneal endothelium cell counts and diopter were measured for phackic anterior chamber intraocular lens(PACL) selection.During the operation,the PACLs were implanted into anterior chamber.The uncorrected/corrected visual acuity (UCVA/CVA),introcular pressure(IOP),corneal endothelium cell counts,anterior chamber flare,PACLs position and pupil reflex and gonioscopy were observed during 2-year follow-up period.ResultsThe average UCVA was 0.73 and CVA was 0.92 in all patients in 2 years after surgery.The average IOP was 14.17 mmHg without high IOP in any case.The average corneal endothelium cells were (2 499.3±319.6)/mm 2 and the loss ratio was 3.4%,and there was no obvious difference from preoperation.The haptic of all the OII phakic 6H lens did not affect the anterior-chamber angle.All the pupils retained good light reflex,and no cataract was found in any case.ConclusionImplantation of PACLs into anterior chamber is a practical method for correction of very high myopia.
