摘要
目的 :探讨下肢深静脉血栓形成的手术治疗方法。方法 :急性下肢深静脉血栓形成 14例 ,先经健侧股静脉置入下腔静脉滤器 ,再切开患侧股静脉 ,远端血栓以挤压法驱出 ,近端髂股静脉内血栓以Fogarty球囊取出 ;球囊扩张左髂静脉狭窄 11例 ,其中 5例放置支架 ;最后做临时性股动静脉瘘。结果 :14例手术成功。 13例随访 2～ 30个月 ,其中 12例患肢肿胀消退 ,1例复发。结论 :本方法创伤小 ,恢复快 ,疗效好 ,多数患者能耐受。
Objective:To discuss the surgical therapy for the lower extremity deep vein thrombosis (DVT).Methods:14 cases of acute DVT employing surgical therapy were enrolled.Vena Cave filters were placed through the health side femoral vein first,followed by dissecting the contralateral popliteal side femoral vein.The thrombuses in the lower extremity veins were extracted by compression whereas,those in iliac veins and femoral veins were extracted by Fogarty catheter.Ballon dilation was performed in 11 left iliac veins,and stents were placed in 5 cases.Finally,temporary femoral arterivenous fistulars were constructed.Results:The procedures succeeded in 14 cases.The Follow-up in 13 cases was conducted for 2～30 monthes.Limb swelling subsided in 12 cases.Thrombosis recurred in 1 case.Conclusion:This method has less trauma and quick recovery and good effect,and most patients could endure it.Placing a temporary vena cava filter is the best choice.
