颈椎侧块钢板内固定术在颈椎后路手术中的应用

摘要 目的 :探讨颈椎侧块钢板内固定术在颈椎后路手术中的应用。方法 :在颈椎后路手术中采用颈椎侧块钢板内固定加植骨融合术治疗颈椎管内肿瘤、颈椎管狭窄症、颈椎骨折脱位共 14例。后正中入路 ,颈椎管内肿瘤及颈椎管狭窄症采用双侧椎板减压 ,颈椎骨折脱位根据情况复位 ,一侧或双侧椎板减压 ,采用Magerl法进钉 ,选用 3.5mm皮质螺钉 ,双皮质固定。结果 :随访 8个月~ 2年 ,平均 14个月 ,术后 3~ 5个月获得良好骨性融合 ,颈椎椎间高度无丢失 ,无螺钉松动及断钉现象 ,脊髓神经功能有不同程度的恢复。结论 :颈椎侧块钢板固定术手术技术相对简单 ,固定节段短 ,力学强度好 ,适应证广 ,是颈椎后路手术的一种有效方法。 Objective: To discuss the application of cervical lateral mass plate fixation in posterior approach of cervical spine operation. Methods: 14 cases of various cervical trauma and disease (including tumors in the cervical spinal canal,stricture of cervical vertebra canal and fractures and dislocation of the lower cervical spine) were treated with posterior cervical lateral mass fixation and bony fusion. The patients received spinal decompression or reduction and decompression according to different disease.A suitable bicortical screw was inserted using Magerl technique. Results: The follow-up of all 14 cases ranged from 8 months to 2 years(with the average of 14 months).All cases obtained solid bony fusion after 3 or 5 months postoperatively and intervertebrai heighs and physiological curves were kept well. No screw loosening or breaking was found. Neurological function had a different recover. Conclusion: Cerical lateral mass plate fixation has the advantages of easily operation technique,wide indications,strong stability and short segment fixation.The results suggestd that this technique is an efficient posterior fixation.
