摘要
目的 :研究胰腺缺血与脂质炎症递质平衡失调在急性水肿性胰腺炎 (AEP)向急性坏死性胰腺炎(ANP)转变中的作用。方法 :10 3只SD大鼠随机分为 :假手术 (SHO)组、AEP组和ANP组三组。AEP组通过胰管结扎、外分泌刺激诱发。在AEP模型同时静注大剂量Dextran 110诱发ANP。测定胰组织及血浆过氧化脂质 (LPO)和血浆血栓素 (TXB2 )及前列腺素 (6 keto PGF1α)。结果 :与AEP组相比 ,ANP组 3、9h胰组织及血浆LPO水平明显增高 ,血浆各时相TXB2 水平亦明显增高 ,而血浆 6 keto PGF1α水平无变化。病理学发现 ,ANP组 9h胰毛细血管内皮极不规则 ,内皮细胞坏死、剥脱 ,基膜破坏。结论 :脂质炎症递质平衡失调可能加重胰腺缺血并促使AEP向ANP转化。
Objective:To investigate the effect of pancreatic ischemia and the disturbance of lipid inflammatory mediator balance on the conversion of acute edematous pancreatitis(AEP) to acute necrotizing pancreatitis(ANP) .Methods:One hundred and three Sprague-Dawley rats were randomized in three experimental groups as follows:Sham-operated group,AEP group,ANP group.AEP was induced by pancreatic duct ligation and exocrine stimulation,ANP was induced by pancreatic duct ligation and exocrine stimulation and intravenous injection of a large dose of dextran-110(500mg/kg).Results:Three and nine hours after induction of pancreatitis,plasma and pancreatic tissue lipid peroxide levels in rats with ANP were markedly higher than that with AEP.In AEP group,plasma thromboxane B2 and 6-keto- PGF 1α remained unchanged.The plasma TXB 2 level increased significantly in ANP group than in AEP group. Ultrastructural examination demonstrated the endothelial cells separated from pancreatic capillary basal membrane and necrotized in ANP group.Conclusion:The disturbance of lipid inflammatory mediator balance may aggravate pancreatic ischemia,induce the conversion of AEP to ANP.
