摘要
目的 :探讨功能性鼻窦内窥镜手术后定期换药治疗的意义。方法 :对220例(412侧)慢性鼻窦炎 ,鼻息肉患者术后定期在鼻窦内窥镜下换药治疗 ,并同期观察鼻腔 ,鼻窦粘膜的转归。结果 :术后1～2周 ,术腔换药方式主要为清除术腔痂皮 ;术后3～12周可见术腔出现囊泡,肉芽,小息肉以及纤维素性粘连等病变 ,此期主要清除这些病变 ;12周以后可见鼻腔 ,鼻窦粘膜逐渐上皮化 ;结论 :术后换药治疗有利于功能性鼻窦内窥镜术后术腔上皮化 ,对减少鼻窦炎症及鼻息肉的复发具有重要的意义。
Objective:To study the significance of periodic dressing change after functional endoscopic sinus sugery.Methods:The periodic dressing change in 220 cases(412 sides) with chronic sinusitis and nasal polyp after functional endoscopic sinus surgery war performed under nasal endoscope,and mucous membrane outcome in operative cavity was observed in the same time.Results:In 1 to 2 weeks after operation,the main mode of dressing change in operative cavity was periodic clearance of crust.In 3 to 12 weeks after operation,the operative cavity developed possiblely a series of pathological changes,such as vesicles,granulation tissue,mini-polys,fibrous hyperplasia,fibrous adhesion and so on,therefore the main mode of dressing change in this stage was to clear above-mentioned pathological changes.The mucous epithelization of operative cavity in nasal and sinus cavity completed gradually in 12 weeks after operation.Conclusion:Periodic dressing change after functional endoscopic sinus surgery is advantageous to the mucous epithelization of operative cavity,it possesses an important significance on reducing the recurrence of sinusitis and nasal polyp.
