摘要
目的 :用动态增强MRI技术了解正常肾脏和梗阻肾脏的形态及功能 ,探讨其诊断梗阻性肾脏病变的价值。方法 :单侧梗阻性肾脏疾病 (肾积水 )患者 8例 ,已被B超或静脉尿路造影确诊。其中成人 2例 ,儿童 6例 ;基于医学伦理 ,选择 8例成年健康人作对照组。运用动态增强MRI技术记录注射增强对比剂 (Gd DTPA)前和后的 1,3 ,5 ,10 ,15 ,3 0 ,45和60min肾皮质、肾髓质和肾盂的区域作为兴趣区 ,计算不同时间点兴趣区的信号强度和相对信号强度 ,对肾脏的形态和功能状态进行评估。结果 :梗阻组与正常组相比 ,肾皮质与髓质和肾盂等部位各时间点的信号强度和相对信号强度以及持续时间均有明显的差异 (P <0 .0 5 )。结论 :动态增强MRI采集肾脏不同时间和区域的信号强度 ,可对肾脏的功能和形态进行联合评估 ,并能了解梗阻肾脏的浓缩稀释和分泌功能、肾功能 ,对于评价儿童和成人肾积水均有重要的价值。
Objective To evaluate renal morphology and function in patients with obstructed kidney by dynamic enhanced MRI.Methods Eight patients with obstructive renal disease (6 children,2 adults) accessed by dynamic enhanced MRI(all single side) were compared with eight healthy controls with normal kidneys (both sides, 16 kidneys).Pre-contrast and post-contrast images were got at 1,3,5,10,15,30,45 and 60 min respectively. Region of interesting(ROI) was selected in cortex, medulla,pelvic.The signal intensity and relative signal intensity were calculated at different time and in different ROI.The morphology and function of kidney were evaluated.Results Compared with control group,the signal intensity of renal cortex,medulla,calyx and duration was obviously different(P<0.05) in obstructed kidney.Conclusion Dynamic enhanced MRI can provide useful information about renal function and configuration, also show sufficient renal concentration, dilution and secretion,and is very useful in diagnosing and treating obstructive renal disease.
