摘要
目的 探讨超声引导下经皮经肝胆管穿刺置管引流 (PTCD)对恶性阻塞性黄疸的临床应用价值及操作方法的改进。方法 回顾性分析 1 1例恶性肿瘤所致的阻塞性黄疸患者 ,应用普通凸阵探头引导行 PTCD术。结果 1 1例共行 1 3次 PTCD,其中 1例行左外叶下段支胆管穿刺失败而改行右前支穿刺 ,其余均一次性穿刺成功 ,手术成功率达 92 .3% (1 2 / 1 3)。术后观察黄疸明显减退者 7例 (6 3.6 % ) ,轻度减退者 4例 (36 .4 % )。结论 普通凸阵探头引导下 PTCD术操作方便 ,手术成功率高 ,疗效确切 。
Objective To study the clinical value of ultrasound guided percutaneous transhepatic cholangiographic drainage(PTCD) in malignant obstructive jaundice.Methods The effects of PTCD guided by ultrasound were summarized restrospectively in 11 cases with malignant obstrctive jaundice.Results Thirteen procedures of PTCD were implemented in the 11 patients.Among 13 procedures,the successful rate was 92 3%(12/13).Among 11 patients,jaundice obviously regressed in 7 patients(63 6%),and mildly regressed in 4 patients (36 4%).Conclusions PTCD guided by ultrasound has the features of convenience with high successful rate.It is worthy to be widely used in clinical practice.
