摘要
目的 研究糖尿病视网膜病变患者血中血栓调节蛋白 (thrombomodulin,TM)的水平以及与眼底视网膜病变的相关性。方法 将糖尿病患者按眼底视网膜病变程度分为 4组 :无糖尿病视网膜病变期 (no diabeticretinopathy,NDR) 30例 ,单纯期 (sim ple diabetic retinopathy,SDR) 30例 ,增生前期 (preproliferative diabeticretinopathy,PPDR) 36例 ,增生期 (proliferative diabetic retinopathy,PDR) 30例 ,以及相应年龄段的健康正常人做为对照组 (control group,CONTRL ) 33例。应用 EL ISA方法检测血浆中 TM的值 ,应用统计学分析 ,t检验进行组间比较。结果 NDR组、SDR组、PPDR组及 PDR组的血浆 TM值分别为 (2 3.97± 7.4 2 ) ng/m l、(2 5 .6 2± 8.33)ng/ml、(2 6 .93± 9.2 1) ng/m l、(32 .5 8± 10 .94 ) ng/ml。对照组值为 (2 2 .12± 6 .86 ) ng/ml。对照组与 PPDR组和 PDR组比较 (P <0 .0 1) ,NDR组与 PDR组比较 (P <0 .0 1) ,SDR组与 PDR组比较 (P <0 .0 1) ,PPDR组与 PDR组比较 (P <0 .0 5 )。结论 糖尿病患者血中 TM水平反映了血管内皮细胞受损伤程度 ,随着内皮细胞损伤程度加重 ,TM值上升 。
Objective To investigate plasma thrombomodulin levels in relation to the severity of different degrees of diabetic retinopathy.Methods The levels of plasma thrombomdulin were examined by the use of ELISA which is called noe-step sandwich enzyme immunoassay.The diabetic patients were divided into four groups according enzyme immunoassay.The diabetic patients were divided into four groups according to the degree of the severity of diabetic retinopathy while the normal people were also checked as the control group.Student's t test was used to analyze the data.Results The levels of plasma thromobomodulin in patients of no diabetic retinopathy,simple diabetic retinopathy,preproliferative diabetic retinopathy,proliferative diabetic retinopathy and control group were (23.97±7.42)ng/ml,(25.62±8.33)ng/ml,(26.93±9.21)ng/ml,(32.58±10.94)ng/ml and (22.12±6.86)ng/ml respectively.The student's t test revealed significant differences between control group and preproliferative retinopathy (P<0.01),between control group and proliferative retinopathy (P<0.01),between no retinopathy and proliferative retinopathy (P<0.01),between simple diabetic retinopathy and proliferative retinopathy (P<0.01),and between preproliferative retinopathy and proliferative retinopathy (P<0.05).Conclusion The levels of plasma thrombomodulin in paitents of diabetic retinopathy increased as retinopathy progressed,ocular vasuclar injuries may correlated with systemic vascular injuries.Therefore,the progress of retinopathy may be predicted by monitoring plasma thrombomodulin levels in selected patients.
