摘要
目的:探讨醒脑静注射液对急性脑出血后意识障碍伴发热患者肿瘤坏死因子-α(tumornecrosisfactor-α,TNF-α)变化的作用及机制。方法:急性脑出血患者40例(醒脑静组)在应用脱水剂、抗生素等处理时,加用醒脑静注射液,并分别于入院时(6h)、第1,2,3,4,5,6,7天采血测TNF-α,并与未用醒脑静注射液的同类对照组患者进行比较,同时观察体温升高幅度与血浆TNF-α含量变化的相关性。结果:醒脑静组与对照组相比,各时间段血浆TNF-α含量均降低,第7天时醒脑静组为(97.40±9.08)ng/L,低于对照组(115.25±10.92)ng/L,差异有显著性意义(t=-3.658,P=0.0007)。醒脑静组平均体温升高和最大体温上升高度明显低于对照组,于第5天基本降至正常范围,而对照组到第7天仍明显高于正常。各组体温升高幅度与血浆TNF-α含量变化呈非常显著的正相关(r=0.981,P<0.001)。结论:醒脑静注射液改善急性脑出血患者意识障碍、降低体温与其抑制TNF-α分泌有关。
AIM:To study the effect of xingnaojing injection on the changes of tumor necrosis factor alfa(TNF α) in patients with consciousness disturbance combined with fever after acute cerebral hemorrhage and its mechanism. METHODS:Forty inpatients with acute cerebral hemorrhage(xingnaojing injection group) were injected with xingnaojing when the dehydration and the antibiotics were adopted.TNF αof patients was measured through blood taking at 6 hours,1,2,3,4,5,6,7 days of hospitalization, which was compared with those of patients of the same kind but without xingnaojing injection in control group,and meanwhile,correlation of the increase range of body temperature with the changes of plasma TNF αcontent was observed. RESULTS:The contents of plasma TNF αin xingnaojing injection group decreased at all time points as compared with those of control group.The content of TNF αin xingnaojing injection group at 7 days of hospitalization[(97.40±9.08) ng/L] was lower than that of control group[(115.25±10.92) ng/L], and the difference was significant(t=-3.658,P=0.000 7).The average increase range of body temperature in xingnaojing injection group and the maximum increase range was significantly lower than that of control group, which decreased to the normal range at the 5th day, while that of control group was still significantly higher than normal range at the 7th day.The increase range of body temperature in each group was in significantly positive correlation with the changes of plasma TNFαcontent(r=0.981,P< 0.001). CONCLUSION:The effects that xingnaojing injection can improve the consciousness disturbance and decrease the body temperature of patients with acute cerebral hemorrhage are relative to its inhibition of TNFαsecretion.
