生物可吸收弹簧圈治疗颅内动脉瘤的初步结果

摘要 目的 评价新型弹簧圈 (Matrix)系统在颅内动脉瘤栓塞治疗中的安全性与效果。方法 应用含有生物可吸收材料的Matrix或结合电解可脱卸弹簧圈 (GDC)栓塞颅内动脉瘤 6 2例。结果  6 2例动脉瘤均成功栓塞 ,其中 2 9例单纯Matrix栓塞 ,33例为Matrix结合GDC栓塞 ,1 5例患者采用支架辅助血管成形术。 4 9例动脉瘤致密栓塞 ,1 3例为大部分栓塞。 1例动脉瘤术中破裂出血 ,2例发生血栓栓塞。 2例患者死亡 ,3例重残 ,其余患者恢复良好。 2例动眼神经麻痹在术后 2周得到明显缓解。 2 5例患者造影随访 (3~ 6个月 )结果稳定 ,无明显载瘤动脉狭窄。结论 Matrix栓塞治疗颅内动脉瘤是有效的 ,可缓解动脉瘤占位效应并预防动脉瘤的复发。 Objective To evaluate the short term efficacy and safety of the new bioabsorbable polymeric coils (Matrix) system. Methods Sixty two patients with intracranial aneurysms were treated with Matrix coil or combined with Guglielmi detachable coils (GDC). Results All 62 cases of aneuryem were successfully embolized including 29 with Matrix alone, while 33 with Matrix and GDC. Total occlusion was achieved in 49 aneurysms and subtotal in 13. One patient experienced with aneurysmal rupture bleeding and two had thromboembolic complications during the procedure. Two patients died, 3 had severe neurologic deficits, while others recovered well in the clinical follow up for 3-5 months. Ocularmotor nerve palsy occurred in 2 cases and resolved 2 weeks later. 3-6 months angiographic follow up in 25 patients showed the result was stable with no stenosis in parent artery. Conclusions Matrix system may release the mass effect and prevent recanalization after endovascular embolization of cerebral aneurysms. Long tesm follow up is necessory for further investigation.
