摘要 目的:观察冠状动脉旁路移植术(CABG)中应用内窥镜采集大隐静脉对减少下肢切口并发症的效果。方法:2003年8月~2004年2月对56例CABG的病人采用内窥镜取大隐静脉,在膝关节中部切一2cm切口,应用VasoView5内窥镜血管采集系统游离获取大隐静脉。结果:平均获取大隐静脉桥血管2.3支,平均耗时(50±20)分钟。全组病人无下肢皮肤切口感染,下肢水肿,淋巴管炎和脂肪坏死等并发症发生。平均卧床时间2~3天。结论:CABG中应用内窥镜采集大隐静脉能够减少创伤,明显降低术后下肢并发症,尤其对肥胖合并糖尿病患者可降低术后感染的发生率,减轻术后下肢切口疼痛,提高病人术后活动能力,减少卧床与住院时间,提高病人的满意度和生活质量。 Objective: To observe the influence of endoscopic harvesting techniques on the prevalence of leg--wound complica- tions after coronary artery bypass grafting.Methods: From August 2003 to February 2004, 56 patients undergoing CABG had saphe- nous vein harvested by endoscopic harvesting system. A puncture incision about 2 cm is made near knee joint proximally, then we divide and harvest the saphenous vein. Results: The mean number of vein grafts of ESVH was 2. 3 and the mean endoscopic harvest time was (50±20) minutes. No complication of incision infection, edema of lower extremity, lymphangitis and fat necrosis happed. Mean ly- ing time was 2~3 days. Conclusion: The use of endoscopic vein harvesting in coronary artery bypass grafting can decreases the preva- lence of postoperative leg--wound infections, postoperative pain, lying time and hospital stay, increases the postoperative mobility abili- ty, patient satisfaction and quality of life, especially for the patient with obesity and diabetes mellitus.
