颞下颌关节疼痛冲洗疗效及关节液中TNFα含量变化 被引量:1

THE CHANGES OF TMJ PAIN AND THE LEVEL OF TNFα IN SYNOVIAL FLUID BEFORE AND AFTER JOINT LAVAGE

摘要 目的 探讨关节上腔冲洗术对颞下颌关节疼痛的影响及疼痛变化与关节液中TNFα含量的关系。方法 在关节上腔冲洗治疗前 ,抽取 4 0名关节源性疼痛TMD患者 4 6侧关节液标本 ,4名患者 6侧关节液标本因血污染被舍弃 ,记录治疗前关节疼痛的VAS值 ;治疗一周后 4 0名患者复诊 36人 ,再次抽取关节液标本 38例 ,并记录VAS值 ;采用双抗体夹心酶联免疫法 (ABC ELASA)检测治疗前后关节液中TNFα的水平。结果 治疗前患者关节液中TNFα检出率为 6 7 5 % (2 7/40 ) ,含量为 (6 4 7 5 6± 4 6 8 75 )pg/mgprotein ,VAS值 :(46 .0 3± 19.6 9)mm ;治疗后患者关节液中TNFα均未检出 ,VAS值 :(17 75± 7 33)mm ,与治疗前相比显著降低。结论 关节疼痛与关节液内TNFα水平密切相关 ;关节上腔冲洗术对缓解患者关节疼痛疗效肯定。 Objective To investigate the influnence of joint lavage on TMJ pain and the relationship between changes of TMJpain and the level of TNFα in synovial fluid. Methods Forty six synovial fluid samples, of which 6 samples with blood contamination were discarded, were obtained and VAS score of TMJ pain was recorded from 40 TMD patients before joint lavage. 38 synovial fluid samples were obtained and VAS score was recorded from 36 patients who had subsequent visit one week after treatment. Enzyme linked immunosorbent assay was used to measure the concentration of TNFα in synovial fluid samples. Results Before treatment, the detectable rate of TNFα in synovial fluid was 67.5%(27/40) and the concentration of TNFα was (647.56±468.75)pg/mg protein and, the VAS score of patients was (46.03±19.69)mm. After treatment, TNFα wasnt detected in all synovial fluid samples and, the VAS score of patients was (17.75±7.33)mm. The pain degree after treatment was significantly lower than that before treatment. Conclusion There is a close relationship between level of TNFα in synovial fluid and TMJ pain, and joint lavage is beneficial to the relief of TMJ pain.
