摘要 目的 探讨基底细胞在前列腺增生 (BPH)发病中的作用。方法 胚胎期前列腺 15例 ,成人非增生前列腺 2 1例 ,增生前列腺 3 4例 ,睾丸去势前列腺 7例 ,去势时间 4~ 7个月。用单克隆 3 4βE12抗体标记基底细胞 ,用增殖细胞核抗原 (PCNA)标记细胞增殖。应用原位缺口平移法检测细胞凋亡。结果 PCNA主要标记基底细胞 ,凋亡成分主要为腺上皮细胞。成人非增生前列腺与增生前列腺上皮细胞增殖、凋亡无显著性差异 (P >0 .0 5 )。在成人非增生前列腺 ,前列腺 3个功能区的形态有一定的差异 ,移行区前列腺可见一些散在的与成熟腺体相邻的细胞团 ,与胚胎期前列腺腺体胚芽结构相似 ,3 4βE12表达阳性。移行区基底细胞的增殖指数明显高于外周区和中央区 (P <0 .0 5 ) ,腺上皮细胞的凋亡指数与中央区相当但低于外周区 (P <0 .0 5 )。结论 基底细胞是前列腺最主要的细胞增殖成分 ,基底细胞的增殖及向腺上皮细胞的分化维持前列腺上皮细胞增殖、凋亡平衡 。 Objective To investigate the roles of basal cells in the pathogenesis of benign prostatic hyperplasia. Methods Of these groups, there were 15 cases of fetal prostate, 21 cases of adult non hypertrophied prostates, 34 cases of benign hypertrophied prostate and 7 cases of castrated prostate with castration time of 4 7 months. Basal cells were immunohistochemically labelled by monoclonal antibody 34βE12. Proliferative indexes were assessed by immunostaining using proliferating cell nuclear antigen (PCNA). Apoptotic cells were specifically stained using in situ nick translation. Results PCNA was mainly expressed in basal cells and apoptotic bodies were mainly in glandular cells. Compared with those of adult non hypertrophied prostates, the proliferative indexes and apoptotic indexes of benign hypertrophied prostates were not significantly different ( P >0.05). In the adult non hypertrophied prostate, there was difference in structures of the three different functional zones. In the transitional zone (TZ) of prostate, 34βE12 positive expression of some dispersive glandular buds and branching formations, adjacent to the mature glandular acinus and similar to the glandular bud in the fetal prostate, was found. Proliferative index of basal cells in the transitional zone was higher than that in the peripheral zone (PZ) or central zone (CZ) ( P <0.05). Apoptotic index of glandular cells in the TZ was similar to that in the CZ and lower than that in the PZ ( P <0.05). Conclusion The proliferative pool is located in the basal cell layer. Proliferation of basal cells and their differentiation into glandular epithelial cells may play a critical role in the balance of proliferation and apoptosis of glandular cells. Imbalance of proliferation and apoptosis of glandular cells in the TZ might be involved in the etiology of benign prostatic hyperplasia.
