摘要
目的内镜下黏膜剥离术(ESD)现已成为消化道早癌及癌前病变的首选治疗方法,在消化道肿瘤的ESD操作中,由于手术钳的作用局限性,可使ESD操作的难度增加,安全性降低新型的SB刀Jr(超小型)是一种使用单极高频电的剪刀式手术钳,其设计和功能的特殊性降低了ESD的操作难度。本研究旨在明确此新型剪刀型SB刀Jr在消化道ESD操作中的功效。方法回顾性选取2013年4月至2015年4月在解放军第八十五医院消化内科经新型剪刀型SB刀Jr行ESD手术切除的消化道肿瘤患者92例,观察消化道肿瘤的部位、切除肿瘤病灶的大小及病理类型、手术时间、并发症等指标进行总结分析。结果 92例患者的肿瘤均被整块切除,切除黏膜的平均大小为38.2 mm,平均操作时间为58.5 min,术中无穿孔、无大量出血或术后迟发性出血等并发症出现。全部ESD操作仅通过新型剪刀型SB刀Jr全部完成73.9%(68/92)。结论新型剪刀型SB刀Jr在消化道肿瘤ESD操作中的功效显著,降低ESD操作难度,同时也增加了其安全性。
Objective Endoscopic submucosal dissection(ESD) is now a new and first-choice therapeutical technique for early gastrointestinal cancer and precancerous lesions,during operation of ESD,endoscopist are confronted with technological difficulty and high risk with usual forceps.Novel stag beetle(SB) knife Jr is a kind of small scissor forcep which using mono-pole high frequency electricity,the particularity of its design and efficiency lower the difficulty and increase safety of ESD.The aim of this study was to evaluate the efficiency of novel SB knife Jr on ESD for gastrointestinal tumors.Methods Thedata of92 gastrointestinal tumors patients who underwent ESD using novel SB knife Jr between April 2013 and April2015 were analyzed retrospectively,incluing the location,pathological pattern,the size of resectedtumors,operating time and complication.Results All tumors(92/92) were resected en bloc.The mean size of the resected tumors was 38.2 mm,the mean operating time was 58.5 min.No complications such as microperforation,massive hemorrhage occurred during the manipulation of ESD.The rates of resection carried out only with SB knife Jr were 73.9%(68/92).Conclusions The using of novel SB knife Jr on ESD for gastrointestinal tumors offers a breakthrough in resection techniques,it not only lowers the difficulty but also increases safety of ESD.
