摘要
目的分析导致肝损伤的药物种类、特点,为临床合理用药提供参考。方法通过'CNKICHKD'中国医院知识仓库,对1994—2012年60种国内公开发表的期刊,进行检索,对患者性别、年龄、原发疾病、导致药物性肝损伤的药物品种及例数、发生时间及给药方式、肝脏损伤情况及治疗转归进行统计、分析。结果男性77例,女性56例,男性多于女性,20~40岁年龄段药物性肝损伤发生率最高;原发疾病涉及系统前三位为循环系统、呼吸系统和内分泌系统;病种前三位为冠心病、肺结核、银屑病;发生肝损伤的药品比例有一定的差异,以抗结核药对肝脏的损伤最大,其次为中草药、循环系统用药、解热镇痛药及抗甲状腺药,用药11~30d发生肝损伤患者最多占30.07%,口服给药发生肝损伤患者最多占73.68%;80.45%的患者停药经保肝治疗后症状完全恢复。结论应该监护药物可能导致的药物性肝损伤,提高用药安全性,减少药品不良反应的发生。
Objective To analyse the species and features of the drug inducing liver injuries,so as to provide the basis to rational use of drngs. Method Through CHKD database from 1994 to 2012 yv. of case report about drug-induced liver injury,to analyse patient's sex,age,primary disease,drug classes delivery ways and incidences of occurring time and severity of induced liver indury. Results Male 77 cases,female 56 cases,male was more than female. Patients aged from 20 to 40yr were prior to drug-induced liver injury. The first three primary system involved by diseases included circulatory,respiratory and endocrine ones; The top three diseases were coronary heart disease,pulmonary tuberculosis and psoriasis. The ratios of drugs which inducing liver injury had certain differences,among which antituberculous drugs impaired liver mostly,Chinese herbal medicines,circulation system drugs,analgesics and antithyroid drugs followed up; 30. 07% of patients liver indury occurred later 11 to 30 days after administration of drugs,73. 68% of patients being of oral administration occurred liver injury,80. 45% of patients recovered completely after hepatoprotective treatment. Conclusion It should be monitoring drugs being whether inducing liver injury in order to improved drug safety and reduce the incidences of ADRs.
