全身炎症反应综合征患者高淀粉酶血症在评估预后中的意义 被引量:5

摘要 目的 :探讨全身炎症反应综合征 (SIRS)患者高淀粉酶血症与炎性细胞因子及预后的关系。方法 :对 83例符合SIRS诊断标准的患者测定血清淀粉酶 ,根据结果分成正常淀粉酶组 (≤15 0U/L)、中淀粉酶组 (15 1~ 4 0 0U/L)和高淀粉酶组 (>4 0 0U/L) ,同时测定肿瘤坏死因子 α(TNF α)、白介素 6 (IL 6 )水平 ,住院观察患者的预后 ,并设健康对照组。结果 :血淀粉酶水平与TNF α、IL 6呈平行关系 ,淀粉酶水平高者多器官功能障碍综合征 (MODS)的发生率高。结论 :血淀粉酶水平与引起SIRS期炎症反应和内皮损伤的炎症介质有关。在SIRS的早期 ,根据血淀粉酶水平的高低评估患者的预后 ,并采取有效的治疗措施 ,减少MODS的发生 。 Objective:To investigate the relationship between hyperamylasemia and inflammatory cytokines as well as its prognosis in patients with systemic inflammatory response syndrome (SIRS). Methods: Eighty-three patients diagnosed as SIRS were divided into normal amylase group (≤150 U/L), moderate amylase level group (151-400 U/L) and high amylase level group(>400 U/L)according to serum amylase level. Simultaneously, levels of tumor necrosis factor-α(TNF-α) and interleukin-6 (IL-6) were measured. Prognosis of each patient was observed in periods of hospitalization.Results: There were positive relationship between serum amylase level and TNF-α and IL-6 level. Patients with hyperamylasemia had a higher risk of developing multiple organ dysfunction syndrome (MODS).Conclusion:Serum Amylase level is related with inflammatory media that will induce inflammatory reaction and endothelial injury in SIRS. Evaluating prognosis of patients with amylase level in early SIRS and take effective management may decrease the risk of developing MODS and increase the cure rate.
