摘要
目的 探讨糖皮质激素 (GC)在急性呼吸衰竭 (ARF)综合救治的临床价值。方法 1 997年 1月至2 0 0 3年 8月 ,对入院的 ARF患者 87例 ,随机分为治疗组 (用 GC组 ) 4 4例与对照组 (不用 GC组 ) 4 3例 ,观察两组性别、年龄、原发病 ,氧合指数 (Pa O2 /Fi O2 ) ,比较病死率 ,分析死亡原因和 GC应用时机、剂量对预后的影响。结果 两组的上述 4项临床指标均无显著差异 (P均 >0 .0 5 ) ,治疗组病死率 38.6 % ,低于对照组 6 0 .5 % (P<0 .0 5 )。结论 在 ARF综合治疗中 ,及时、适量应用 GC有助于改善缺氧和休克 ,减少
Objective To explore the clinical effects of glucocorticoid (GC) in the treatment of acute respiratory failure (ARF).Methods Eighty seven cases with ARF were randomly divided into two groups: GC and non GC groups, during January 1997 to August 2003. Their sex, age, primary diseases, PaO 2/FiO 2, level of positive end expiratory pressuer (PEEP) and mortality rate were compared between the two groups. The opportunity of administration on GC, the dosage, and the duration of treatment were compared between the groups. Results The four clinical items showed no significant difference between the two groups (p>0.05). The mortality rate for patients who were treated with GC was lower than those without (38.6% VS 60.5%) (p<0.05).Conclusion GC could be one of the effective treatments for ARF. Proper dosage of GC should be given without hesitation when refractory hypoxemia and shock are present.
