糖尿病性血管病变病人胰岛β细胞早期分泌相的变化

摘要 1目的 探讨 型糖尿病合并慢性血管病变病人胰岛 β细胞早期分泌相的变化规律。 2方法 采用放射免疫法分别检测了 85例病人空腹及左旋精氨酸 (L- ARG)刺激后 2 ,4,6 m in时的胰岛素 (INS)、C肽 (C- P)水平的变化 ,并与 6 0例健康人进行对照。 3结果  型糖尿病合并血管病变组空腹胰岛素明显高于对照组和无血管病变组 (t=2 .15 ,4.15 ,P<0 .0 5 ,0 .0 0 1) ;L- ARG刺激后对照组和 型糖尿病两组 INS和 C- P分泌均在 2 m in时达峰值 , 型糖尿病合并血管病变组 2 ,4,6 min时的 INS分泌值及 2 ,4min时的 C- P分泌值均明显高于无血管病变组(t=2 .0 1~ 3.2 3,P<0 .0 5 ,0 .0 1) ;肥胖糖尿病合并血管病变组空腹 INS及 L- ARG兴奋后 INS,C- P峰值均明显高于非肥胖组 (t=2 .6 5~ 4.98,P<0 .0 5 ,0 .0 1)。 4结论 有血管病变的糖尿病病人存在较严重的胰岛素抵抗 (IR) ,伴有肥胖者更明显 ;IR及胰岛 β细胞早期分泌功能的紊乱与糖尿病性血管病变的发生。 Objective To investigate the changes of isletβ-cell early secretory phase in patients of diabetes millitus type complicated with diabetic angiopathy. Methods The serum insulin(INS) and C-peptide(C-P) levels were measured in85 pa- tients with type diabetes mellitus and 6 0 healthy subjects at fasting,2 ,4and 6 min after being injected L-ARG with radioim- munoassay. Results The fasting INS levels were significantly higher in patients with diabetic angiopathy than those in controls and non angiopathy patients(t=2 .15 ,4.15 ,P<0 .0 5 ,0 .0 0 1) ;The INS and C-P levels reached the highest 2 m in after L -ARG in- jection. The INS levels at 2 ,4,6 min and the C-P levels at 2 ,4min after L -ARG injection were significantly higher in patients with diabetic angiopathy than those without angiopathy(t=2 .0 1— 3.2 3,P<0 .0 5 ,0 .0 1) ;The fasting INS and the maxim al INS,C-P levels after L -ARG injection were significantly higher in patients with diabetic angiopathy accom panied by obesity than those with- out obesity(t=2 . 6 5— 4.98,P<0 .0 5 ,0 .0 1) . Conclusion There were severe insulin resistance(IR) in patients with diabetic angiopathy,especially those accom panied by obesity;IR and the disorder of isletβ-cell early secretory function might be related with the occurrence and development of diabetic angiopathies.
