摘要
全部进行了TCD及BAEP检查的286例VBI患者,症状主要有与颈部活动有关的眩晕,耳鸣、行走不稳、呕吐等,体征以眼震、共济失调、腱反射活跃等为主,病因主要有颈椎病、动脉硬化、高脂血症、血粘度增高及高血压。TCD总异常率88.46%(发作期96.15%,缓解期84.07%),BAEP总异常率80.77%(发作期93.1%,缓解期72.35%),两者发作期异常率均显著高于缓解期;两项检查比较,总异常率及发作期异常率无显著差异,缓解期TCD异常率明显高于BAEP。认为TCD与BAEP从不同角度反映VBI的循环及电生理变化,两者结合对VBI的诊断及疗效观察有肯定价值。
286 cases with vertebrobasilar blood insufficiency (VBI) were examined by transcranial doppler (TCD) and brains tern auditory evoked potential (BAEP).The main symptoms were vertigo related with neck movement,tinnitus,gait disturbances,vomiting.The main signs were nystagmus,ataxia,and increased tendon reflexes.The cause of VBI were cervical spondylosis,arteriosclerosis,hyperlipemia,hyperviscositemia and hypertension.Total abnormal rate of TCD was 88.46% (stage of attack 96.15%.silent period 84.07%).Total abnormal rate of BAEP was 80.77% (stage of attack 93.1%,silent period 72.35%).Abnormal rate of TCD and BAEP in stase of attack were significant higher than that in silent perild.In silent period,the abnormal rate of TCD was significant higher than that of BAEP,but in stage of attack,total abnormal rate have no statisticall significance.We think that TCD and BAEP can reflect change of the circulation and electrophysiology of VBI.The combination of two methods would help to diagnosis and treatment of VBI.
