摘要
目的探讨2型糖尿病患者主动脉钙化与左室舒张功能减退以及与反映血管功能的脉搏波传导速度(PWV)及踝肱指数(ABI)的关系。方法入选2008年4月至2009年4月福建医科大学附属第二医院128例2型糖尿病患者,根据胸部X线摄片有无胸主动脉钙化将患者分为两组。应用彩色超声心动图测定左室舒张早期E峰速度峰值(E)、舒张晚期A峰速度峰值(A)并计算E/A,E/A<1为左室舒张功能减退。应用动脉硬化测定仪检测脉搏波传导速度(PWV)及踝肱指数(ABI)。测定两组患者血压、体重指数、空腹血糖、糖化血红蛋白(HbA1c)、血脂。结果 2型糖尿病患者主动脉钙化与左室舒张功能减退相关,优势比OR=3.325(1.4245~7.7610)。两组脉搏波传导速度(PWV),PWV-R(右侧)分别为:(1989.50±400.68)和(1899.89±1419.15)cm/s;PWV-L(左侧)分别为:(2005.67±419.3)9和(1635.63±252.97)cm/s,差别均有统计学意义。结论 2型糖尿病患者主动脉钙化是左室舒张功能减退的危险因素,同时提示血管壁僵硬度增高。
Object Some studies indicated that vascular calcification was associated with cardiac dysfunction and vascular function.The study was designed to investigate the relationship between aortic calcification with left ventricular diastolic function(LVDF) and aortic stiffness which was estimated by brachial-ankle pulse wave velocity(PWV) and ankle brachial index(ABI) in type 2 diabetes(T2DM).Methods 128 cases of T2DM were involved in the study.Plain X ray was applied to detect the aortic arch calcification.Color M-mode Doppler echocardiography was applied to detect peak flow velocity of early diastole(E) and peak flow veloeity of atrial contraction(A),then E / A was caculated.PWV and ABI were measured with a volume-plethysmographic apparatus.Results In type 2 diabetes aortic calcification was related to impaired LVDF[OR=3.325(CI=1.4245-7.7610)].There was significantly different between aortic calcification group and non-calcification group(PWV-R:(I989.50±400.68)cm/s vs(1899.89±1419.15)cm/s;PWV-L: 2005.67±419.39 vs(1635.63±252.97)cm/s,P<0.05).Conclusion In T2DM aortic calcification was associated with impairment of left ventricular diastolic function and arterial stiffness.
