摘要
目的探讨合理应用单纯尿道扩张、筋膜扩张器尿道扩张以及尿道狭窄段绿激光汽化术治疗尿道狭窄或闭锁的临床方法。方法回顾性分析2009年3月～2010年1月收治的48例男性尿道狭窄或闭锁患者的治疗过程。结果所有患者均成功完成治疗,无尿道穿孔、大出血或直肠损伤等并发症发生。46例患者获随访6～14个月,28例(60.9%)患者拔除尿管1月后排尿通畅,最大尿流率(17.6±5.4)ml/s,18例(39.1%)拔管后6～10天开始出现尿线变细,经定期尿道扩张等后续治疗后症状改善明显。结论根据患者尿道软镜检查结果来选择合理的尿道扩张治疗或腔内瘢痕切除手术联合尿道扩张治疗,是处理尿道狭窄或闭锁的操作性强、成功率高且安全有效的治疗方法。
Objective To investigate a safe and effective method for the treatment of urethrostenosis or urethral atresia.Methods 48 cases with urethrostenosis or urethral atresia were treated by direct urethral dilation for 8 cases,fascia dilator urethral dilation for 17 cases and laser vaporization for 23 cases respectively.Results All the treatment were successfully completed without any urethra perforation,hemorrhoea or rectal injury. During the follow-up of 6～14 months,28 patients were treated successfully and voided fluently 1 month after removal of the catheter.18 patients voided unfluently 6～10 days after removal of the catheter,and then obtained satisfactory results after regular urethral dilation.Conclusions The urethral dilation and laser vaporization are safe and effective methods for the treatment of urethrostenosis or urethral atresia.But it's important to choose one or more reasonable treatment according to the urethrascope examination.
