摘要
目的分析新兵常规12导联心电图的特征及常见图形表现,为新兵能否进一步参加高强度训练提供参考。方法对8 621例新兵做常规12导联心电图检查,将心电图检查结果分类进行分析。结果正常心电图7 038例(82%),窦性心动过缓957例,窦性心律不齐264例,窦性心动过速30例,左心室高电压142例,T波波幅增高57例,一度房室阻滞17例,(不)完全性右束支阻滞28例,室性期前收缩23例,室上性期前收缩27例,ST-T改变11例,游走心律10例,早复极波15例。其中9例经心得安试验ST-T恢复正常,948例窦性心动过缓者运动后心率>90次/min,27例窦性心动过速者在休息心理疏导后心率<100次/min,28例不完全性右束支阻滞、完全性右束支阻滞经心脏彩超检查排除先天性心脏病,57例T波波幅高大与神经功能失调相关,142例左心室高电压经心脏彩超检查排除心室肥大。结论新兵入伍前经过严格筛选,故常规十二导联心电图检查多表现正常及正常范围心电图,部分新兵因训练强度大,心理适应能力、生活方式改变等会造成心电图图形的改变,因此在判断其异常心电图及临床意义时应审慎,以免造成心理压力,也为部队以后的训练做好风险评估。
Objective To analyze the characteristics and common graphics performance of routine 12-lead electrocardiogram of the recruits.Methods 8 621 cases of recruits for routine 12-lead electrocardiogram examination were involved.And the electrocardiogram results were classified.Results Normal electrocardiogram were recorded in 7038 cases(82%),sinus bradycardia in 957 cases,sinus arrhythmia in 264 cases,sinus tachycardia in30 cases,left ventricular high voltage in 142 cases,increased T wave in 57 cases,I degree atrioventricular block in17 cases,complete or incompleteright bundle branch block in 28 cases,ventricular premature contracts in 23 cases,superventricular premature contracts in27 cases,ST-T change in 11 cases,wandering rhythm in 10 cases,early repolarization in 15 cases.Among them 9 cases by propranolol test ST-T returned to normal,948 cases with sinus bradycardia after exercise hear rate became above 90 beats/min,28 cases with incomplete or complete right bundle branch block by ultrasonic cardiogram examination rule out congenital heart disease,57 cases with T wave amplitude tall associated with neurological dysfunction,142 cases with left ventricular high voltage examined by ultrasonic cardiogram to exclude ventricular hypertrophy.Conclusions Because the recruits had been screened strictly before,most conventional 12 lead electrocardiograms presented normal.But on account of heavy training intensity,psychological adaptability,lifestyle changes will cause the change of electrocardiogram,in the judgment should be prudent when abnormal electrocardiogram and its clinical significance,so as not to cause the recruits psychological pressure,and risk assessment for the troops after training.
