摘要
目的 探讨心脏缺血损伤标志物心型脂肪酸结合蛋白(heart type-fatty acid-binding protein,H-FABP)和糖原磷酸化酶BB(glycogen phosphorylase BB,GPBB)检测在招飞体检中的应用价值. 方法 80名参加招飞体检的学生,按心电图正常与否分为两组.心电图异常组:静息和平板运动后心电图异常者50人;对照组:心电图正常者30人.采用酶联免疫吸附法检测招飞学生平板运动试验前和运动后即刻、8h、24 h血液H-FABP和GPBB浓度.采用重复测量方差分析比较两组结果,组内两两比较采用SNK检验. 结果 两组招飞学生血清H-FABP和GPBB浓度平板运动试验前后差异有统计学意义(F=29.000~67.494,P<0.01),运动后较运动前均显著升高,以运动后8h水平最高,24 h后有所回落.无论运动前还是运动后各时间点,两组间血清H-FABP和GPBB浓度差异均有统计学意义,心电图异常组显著高于对照组(F=4.100~23.051,P<0.01或0.05).结论 H-FABP、GPBB血液浓度检测反映了平板运动时心肌利用供能物质和细胞膜通透性的变化,可为招飞体检心脏功能评定提供参考.
Objective To evaluate the application value of finding cardiac ischemic injury markers of heart-type fatty acid-binding protein (H-FABP) and glycogen phosphorylase BB (GPBB) in the physical examination of pilot recruitment.Methods In the physical examination 80 pilot candidates were divided into 2 groups by electrocardiogram results.Fifty candidates were included in abnormal group with abnormal electrocardiogram both in resting and after treadmill exercise and 30 candidates were included in the control group with normal electrocardiogram.Serum H-FABP and GPBB concentration of those candidates were detected by enzyme-linked immunosorbent assay before,immediately after,8 h and 24 h after treadmill exercise test.Results were analyzed by repeated variance analysis and SNK test.Results Levels of serum H-FABP and GPBB of two groups after treadmill exercise were significantly higher than those of before exercise (F=29.000~67.494,P<0.01).The levels appeared peak at 8 h after exercise and fell after 24 h.Significant difference in the concentration of serum GPBB and H-FABP was found between those two groups at all test time points,and the levels of abnormal electrocardiogram group were significantly higher than those of control group (F=4.100~23.051,P<0.01 or 0.05).Conclusion The variations of serum HFABP and GPBB concentrations reflect the changes on candidates' myocardial energy substances and cell membrane permeability in treadmill exercise and these would provide reference for evaluating candidate's cardiac function in the physical examination.
