超声造影定量分析颈动脉斑块内增强强度对照研究 被引量:11

Quantitative contrast-enhanced ultrasound analysis of carotid plaque neovascularization in relation to semiquantitative analysis

摘要 目的利用超声造影定量分析计算颈动脉斑块内增强强度(EI),同时采用半定量分析对颈动脉斑块增强进行分级,比较这两种方法的结果是否一致。方法颈动脉粥样硬化患者135例,共167个颈动脉斑块,其中双侧颈动脉病变患者32例。超声造影观察斑块内有无增强并将斑块增强分为0~4级。随后用Qlab定量分析软件分别计算斑块内EI及斑块内EI与颈动脉管腔内EI比值Ratio。比较不同超声造影增强分级颈动脉斑块内EI及Ratio值差异。结果 0~4级不同超声造影增强分级的颈动脉斑块其超声造影定量分析的EI分别为(1.42±2.15)、(2.60±2.64)、(3.77±2.37)、(4.63±3.32)、(5.99±3.10)dB,Ratio值分别为0.09±0.11、0.15±0.12、0.22±0.11、0.26±0.17、0.34±0.14,差异均有统计学意义(F值分别为13.534、18.647,P均<0.0001),相邻超声造影增强分级的颈动脉斑块EI及Ratio值差异无统计学意义。结论超声造影增强分级与定量分析EI两种方法评价劲动脉斑块内增强程度具有一定的一致性,实际应用中可根据不同需要及本身具备的条件选择合适的方法。 Objective To analyze the relation between the degree of carotid plaque enhancement visualized by contrast-enhanced ultrasound(CEUS) and quantitative parameters of enhancement intensity in the plaques.Methods Conventional ultrasound and CEUS were performed on 135 patients in which bilateral atherosclerotic lesions were detected in 32 patients.Therefore,a total of 167 carotid arteries and plaques were documented and analyzed.Sonographic examinations were performed with ultrasound instruments equipped with low mechanical index contrast imaging software.Enhancement within the plaque was categorized as grade 0 to grade 4 by visual interpretation and was evaluated by quantitative analysis.The enhanced intensity(EI) of the plaque and the ratio of EI within the plaque to that in the lumen of the carotid artery were calculated.Results EI was(1.42±2.15),(2.60±2.64),(3.77±2.37),(4.63±3.32) and(5.99±3.10) dB,respectively in carotid plaques corresponding to grades 0-4 CEUS enhancement.Accordingly,ratio was 0.09±0.11,0.15±0.12,0.22±0.11,0.26±0.17 and 0.34±0.14,respectively.EI(F=13.534,P<0.0001) and ratio(F=18.647,P<0.0001) were significantly different between plaques of different CEUS enhancement grades,respectively.EI and ratio were not significantly different between groups of carotid plaques with similar CEUS grades.Conclusions The degree of carotid plaque enhancement evaluated with quantitative analysis is correlated with that with semiquantitative assessment.The quantitative and semiquantitative methods provide us different choice to achieve different clinical purpose.
