子痫前期与代谢的相关性研究 被引量:8

摘要 目的探讨子痫前期(PE)与基础血压、体脂参数、孕期糖代谢参数、脂代谢参数之间的相关性。方法选取2008年1月至2011年12月在广东省妇幼保健院产前检查并住院分娩的PE孕妇309例为研究组,同期正常孕妇1245例为对照组,对两组的基础血压、体脂参数、脂代谢参数、糖代谢参数进行研究,并分析妊娠晚期入院收缩压(ASBP)、入院舒张压(ADBP)、入院平均动脉压(AMAP)与孕前体重指数(Pre-BMI)、基础收缩压(BSBP)、基础舒张压(BDBP)、血脂、空腹血糖(FPG)、胰岛素抵抗指数(HOMA-IR)等的相关性。结果 PE组与正常组孕妇BSBP分别为(122.03±11.33)mmHg和(109.74±9.40)mmHg,BDBP分别为(75.06±8.06)mmHg和(67.37±6.01)mmHg,Pre-BMI分别为(22.74±3.37)和(20.10±2.07),FPG分别为(4.62±0.95)mmol/L和(4.27±0.52)mmol/L,HOMA-IR分别为(2.13±1.52)和(1.75±0.97),血甘油三酯(TG)分别为(3.91±1.75)mmol/L和(2.76±1.14)mmol/L,总胆固醇(TC)分别为(6.29±1.63)mmol/L和(5.71±1.14)mmol/L、游离脂肪酸(FFA)分别为(769.46±351.46)mmol/L和(551.11±240.45)mmol/L、低密度脂蛋白(LDL)分别为(2.98±1.04)mmol/L和(2.85±0.82)mmol/L,高密度脂蛋白(HDL)分别为(1.42±0.37)mmol/L和(1.51±0.36)mmol/L,分别比较,PE组孕妇基础血压、Pre-BMI、FPG、HOMA-IR、TG、TC、FFA、LDL明显高于正常组,HDL明显低于正常组,差异均有统计学意义(P<0.05)。FPG、TG、TC、FFA、HOMA-IR、Pre-BMI、基础血压均与孕晚期血压显著相关(P<0.05)。进一步应用多元逐步回归分析发现BSBP、孕周、TG、Pre-BMI、FFA、FPG、TC进入AMAP的回归方程,回归系数分别为0.425,-1.295,1.580,0.941,0.008,2.330,0.772,P<0.05。结论 BSBP、Pre-BMI、FPG、TG、TC、FFA与妊娠晚期血压紧密相关,是子痫前期的良好预测因子。 Objective This study is to explore the relationship between metabolic markers and Preeclampsia(PE).Methods 309 patients with PE and 1,245 women with uncomplicated pregnant were selected in this study.The main outcome measures included Pre-gestational Body Mass Index(Pre-BMI),Blood Pressure,Triglyceride(TG),Total Cholesterol(TC),Low Density Lipoprotein(LDL),High Density Lipoprotein(HDL),Free Fatty Acid(FFA),Fasting Plasma Glucose(FPG),HOMA-IR.Statistical analysis was performed with the SPSS 16.0 software.Results As expected,patients with PE had significantly higher levels of Basis Systolic Blood Pressure(122.03±11.33mmHg vs 109.74±9.40mmHg),Basis Diastolic Blood Pressure(75.06±8.06mmHg vs 67.37±6.01 mmHg),Pre-BMI(22.74±3.37 vs 20.10±2.07),TG(3.91±1.75mmol/L vs 2.76±1.14mmol/L),TC(6.29±1.63mmol/L vs 5.71±1.14mmol/L),LDL(2.98±1.04mmol/L vs 2.85±0.82mmol/L),FFA(769.46±351.46mmol/L vs 551.11±240.45mmol/L),FPG(4.62±0.95mmol/L vs 4.27±0.52 mmol/L),and HOMA-IR(2.13±1.52 vs 1.75±0.97),But the level of HDL level was lower in PE(1.42±0.37mmol/L vs 1.51±0.36 mmol/L),P<0.05.Levels of FPG,TG,TC,FFA,HOMA-IR,Pre-BMI,Basis Systolic Blood Pressure(BSBP) and Basis Diastolic Blood Pressure(BDBP) were significantly negatively correlated with blood pressure in late pregnancy(P<0.05).Further application of stepwise multiple regression equation analysis showed that BSBP,pregnant week,TG,Pre-BMI,FFA,FPG,TC came into the regression equations of Admission Mean Arterial blood Pressure(AMAP),the regression coefficient was 0.425,-1.295,1.580,0.941,0.008,2.330,0.772,respectively,P<0.05.Conclusion FPG,Pre-BMI,TG,FFA,TC,BSBP,which were closely related to blood pressure in late pregnancy,were good predictors of preeclampsia.
