摘要 目的探索超声引导下局部注射氨甲蝶呤(MTX)治疗输卵管异位妊娠,并与肌内注射MTX的临床疗效进行比较。方法选择本院早期未破裂型输卵管异位妊娠患者63例,遵照患方自愿的原则非随机分为A组36例和B组27例。A组采用局部注射MTX,即选用20G长200 mm的穿刺针,在3.5 MHz超声引导下向异位妊娠包块内穿刺注入100 mg MTX;B组采用肌内注射MTX0.4mg/kg×5 d。结果 A组治疗成功率为94.4%,显著高于B组的74.1%;A组的药物毒副反应显著低于B组。结论超声引导下局部注射MTX治疗未破裂输卵管妊娠是一种较安全、可靠的方法,且毒副作用小,其治疗效果优于肌内注射MTX。 Objective To study ultrasound-guided local injection of methotrexate(MTX) in treatment of tubal ectopic pregnancy,and to compare the clinical effects of local injection and intramuscular injection.Methods Sixty-three cases with early and unruptured tubal ectopic pregnancy were chosen,and divided into A(36 cases) and B(27 cases) groups.Group A received local injection of MTX.20 G(length 200 mm) needle was guided by ultrasound(3.5 MHz) to inject 100 mg MTX into ectopic pregnancy sac.Group B received intramuscular injection of MTX 0.4 mg/kg for 5 d.Results The treatment success rate in group A was 94.4%,significantly higher than 74.1% in group B.The side-effects in group A were significantly less than those in group B.Conclusion The local injection of MTX guided by ultrasound to treat unruptured tubal ectopic pregnancy is a relatively safe and reliable method,with less side-effects and better curative effects than intramuscular injection of MTX.
