摘要
目的:观察舒芬太尼联合丙泊酚用于无痛人流的临床效果及对人流术后宫缩痛的影响。方法:150例ASAⅠ~Ⅱ级行无痛人流术的早孕妇女,随机分为对照组(P组)、芬太尼组(F组)和舒芬太尼组(S组)(n=50)。静注丙泊酚前5 min分别施以不同麻醉药物。监测患者麻醉前、意识消失即刻、给药(丙泊酚)后2 min及术毕即刻4个时点的平均动脉压(MAP)、心率(HR)、脉氧饱和度(SpO2);记录术后宫缩痛的VAS评分及不良反应情况。结果:F组和S组在同一时间点术后宫缩痛VAS评分均低于P组(P<0.05,P<0.01)。与苏醒后5 min比较,F组和S组从苏醒后15 min开始VAS评分即逐渐降低(P<0.05),而P组则从苏醒后30 min才开始降低(P<0.05)。苏醒后45 min的VAS评分S组小于F组(P<0.05)。结论:舒芬太尼能有效抑制人流术后宫缩痛,镇痛效果要优于芬太尼,苏醒时间短。舒芬太尼联合丙泊酚是无痛人流麻醉较好的伍用选择。
Objective To evaluate the clinical efficacy of sufentanil combined with propofol on painless artificial abortion and the postoperative uterine contraction pain.Methods Total 150 ASAⅠ~Ⅱ early pregnant women who voluntarily received painless artificial abortion were randomly divided into 3 groups(n=50 each):Group P,Group F and Group S.Different drugs were respectively given to the patients five minutes before propofol injection.The following items were recorded and observed such as MAP,HR,SpO2,Pain of VAS score after artificial abortion,and the adverse reactions before anesthesia,the moments of unconsciousness,2 minutes after propofol injection and at the end of operation,respectively.Results Pain of VAS score after artificial abortion in group F and group S were lower than that in group P at the same time(P<0.05 or P<0.01).Pain of VAS score after artificial abortion in group F and S began to decrease from 15 minutes after operation(P<0.05),while the pain of VAS score after artificial abortion in group P began to decrease from 30 minutes after operation(P<0.05).Pain of VAS score after artificial abortion in group S was lower than that in group F from 45 minutes after operation(P<0.05).Conclusion Sufentanil can effectively inhibit the uterine contraction pain after painless artificial abortion,and provide the better analgesic effect and the shorter recovery time than fentanil.Sufentanil combined with propofol is the better compatible selection for anesthesia of painless artificial abortion.
