摘要
目的:总结我院及文献资料冠状动脉造影术(CAG)并发症发生率及防治经验。方法:Seldingers法穿刺股动脉,用标准Judkins法行选择性左、右冠状动脉造影及左室造影。记录术中、术后出现的各种并发症。主要并发症定义为死亡、急性心肌梗塞、急性心力衰竭、严重心律失常及血管并发症等。检索国内1999年以前有关CAG并发症方面的主要文献,综合分析比较CAG主要并发症发生率。结果:535例患者手术成功率99.8%。心室颤动发生率0.37%(2/535),严重缓慢性心律失常发生率1.31%(7/535),血管并发症发生率1.31%(7/535),无死亡、急性心肌梗塞、急性心力衰竭及动脉栓塞、夹层等严重并发症。尽管本组离子造影剂使用率达42.1%,主要并发症发生率2.99%(16/535),与文献资料比较3.01%(141/2467)并无明显差异。结论:冠状动脉造影术有出现严重并发症可能,但通过有效防治,其安全性临床是可以接受的。
Objective:To summarize the complications of 535 patients' coronary artery angiograms, and to compare with collected data from literature to show safty and the ways to prevent untoward effects. Methods:535 patients underwent selective coronary artery angiogram with standard Judkins method, and all complications were recorded during and after the procedures. Death, acute myocardiol infarction, acute heart failure, severe arrhythmia, and peripheral vascular complications were the main complications. All the complications rccorded in the literature before 1999 had been reviewed, and the data were analysed, then compared carefully. Results: The successful rate of the procedure in 535 patients was 99.8%, ventricular fibrillation occurred in only 0.37%(2 of 535) of patients, peripheral vascular complications occurred in 1.31% (7 of 535). Severe bradycardia occurred in 1.31%. There were no other complications including death, AMI, heart failure, artery embolism,or dissection aneurysm in this group, even with the utilization of ionic contrast in as high as 42.1% of patients in this group. The main complication rate was 2.99%, compared with 3.01% reported in the literature, showing that, there was no obvious difference. Conclusion:There is a possibility of severe complication in coronary artery angiogram. With effective prevention, its safaty was acceptable clinically. Ionic contrast did not influence the incidence of main complications.
