摘要
目的:探讨外固定器在常见四肢显微外科手术中的适应症、一般原则及应用技术。方法:依据不同的四肢显微外科手术,选择合适类型的外固定器、标出外固定器的进针部位,确定显微外科手术与安装外固定器的顺序。结果:上肢断肢再植1例、四肢大血管损伤11例、四肢火器伤8例、四肢骨缺损3例、开放性四肢骨折17例、小腿平行桥接游离皮瓣移植3例,均应用外固定器固定骨骼及关节。不但简化了手术操作,缩短了手术时间及移植组织缺血时间,而且稳定性好,固定确实,调整方便。结论:外固定器应用于四肢显微外科手术具有内外固定的优点,克服了内外固定缺点,扩大了四肢修复重建的适应症。
Objective: To evaluate the indications, technical principles, and surgical procedures of using external fixators in microsurgery in extremities. Methods: Surgical procedure, including the selection of appropriate type of external fixator, the determination of the insertion points of pins, and the priority of installation of the fixator were evaluated for the use of external fixator in various microsurgical procedures. Results: The external fixators were used in the replantation of the upper limb in one case, major vascular injury of extremities in 11 cases, gunshot wound of the extremities in 8 cases, repair of bone defect in 3 cases, open fractures in 17 cases, and vascularized parallel-bridging of skin flaps in 3 cases. The results revealed that application of external fixator in microsurgery could not only simplify the surgical procedure, but also shorten the operation time and the ischemic time of transplanted tissues, with such additional advantages as stable fixation and easy adjustment. Conclusion: The application of external fixator in microsurgery in the extremities has the advantages of both conventional internal and external fixation without their disadvantages, and the indications for reconstruction surgery in the extremities could be widened.
