血压晨峰与原发性高血压患者的心脏重构和心律失常的相关性研究 被引量:5

The relationship between morning blood pressure surge and left ventricular hypertrophy and cardiac arrhythmia in patients with essential hypertension

摘要 目的研究原发性高血压患者血压晨峰与左心室肥厚(LVH)以及心律失常的相关性。方法原发性高血压患者193例,根据动态血压检测(ABPM)结果,分为晨峰组(73例)和非晨峰组(120例),同步记录24h动态血压和动态心电图,分析24h动态血压参数、24h动态心电图检出的房性、室性心律失常;超声技术检测左心室质量指数(LVMI)、左心房内径(LAD)等指标,分析两组上述指标的差异。结果晨峰组与非晨峰组比较:LVMI分别为(117.9±19.6)g/m2vs.(93.5±11.6)g/m2,P<0.01;LAD分别为(46.1±10.4)mmvs.(38.4±8.8)mm(P<0.05);左心室肥厚(LVH)的检出率分别为64.4%vs.30.8%(P<0.01)。房性早搏检出率分别为91.8%vs.81.7%(P<0.05);房性心动过速检出率分别为54.8%vs.20.8%(P<0.01);心房颤动检出率分别为23.3%vs.0(P<0.01);室性早搏和室性心动过速的检出率分别为76.7%vs.55.8%(P<0.05)和5.5%vs.2.5%(P<0.05)。44%的心律失常发作出现在06:00~12:00时间段,其中53%发作出现在清晨(06:00~08:00)。结论与无血压晨峰的高血压病患者比,具有血压晨峰的高血压患者左心室肥厚更显著,心律失常更多见,且多出现在清晨。 Objective To investigate the relationship between morning blood pressure surge(MBPS) and left ventricular hypertrophy and cardiac arrhythmia in patients with essential hypertension.Methods 193 patients with hypertension submitted to 24 hours ambulatory blood pressure monitoring and Holter electrocardiographic monitor of 24 hours at the same time were categorized as MBPS group(n= 73) and non-MBPS group(n=120).Left ventricular mass index(LVMI) and left atrial dimension(LAD) were measured by utrasonogragphy in all patients.Results Compared with the non-MBPS group,the MBPS group had significantly higher LVMI [(117.9±19.6)g/m2 vs.(93.5±11.6)g/m2,P<0.01],LAD [(46.1±10.4)mm vs.(38.4±8.8)mm,P<0.05] and LVH(64.4% vs.30.8%,P<0.01);atrial premature beats detected were 91.8% vs.81.7%(P<0.05),atrial tachycardia detected were 54.8% vs.20.8%(P<0.01);atrial fibrillations were 23.3% vs.0%(P<0.01);ventricular premature beats and ventricular tachycardia were 76.7% vs.55.8%(P<0.05) and 5.5% vs.2.5%(P<0.05),44% of cardiac arrhythmia occurred at 06:00-12:00 and 53% at 06:00-08:00.Conclusions Hypertensive patients with MBPS have been shown to have more severe LVH and enlarger LAD and more severe cardiac arrhythmias than that of hypertensive patients with non-MBPS.
