伽玛刀与射频热凝术治疗原发性三叉神经痛比较 被引量:2

摘要 目的探讨伽玛刀与射频热凝术治疗原发性三叉神经痛的疗效。方法 2003年1月~2008年3月,我院应用伽玛刀治疗原发性三叉神经痛80例;射频热凝术治疗三叉神经痛60例。疗效评估采用疼痛的控制程度来观察治疗的疗效。结果平均随访期22±12.5个月。伽玛刀治疗一年有效率78例(97.5%);二年有效率77例(96.3%)。而射频热凝术治疗一年有效率58例(96.8%);二年有效率45例(75%)。两种疗效比较伽玛刀治疗优于射频热凝术(P<0.01)。两种治疗方法的并发症,面部麻木、感觉障碍,射频热凝术明显高于伽玛刀治疗(P<0.01)。结论伽玛刀治疗原发性三叉神经痛能够显著缓解疼痛、提高生活质量,不良反应发生较低。射频热凝术只要技术准确,效果肯定,但有一定并发症;虽复发率高但可重复治疗,治疗费用低。两种疗法的评估有待进一步研究。 Objective To evaluate the therapeutic efficacy of gamma knife radirosurgery(GKS)and radiofrequency thermolesion(RT)for the treatment of prinary trigeminal neuralgia(TN).Methods A comparison of 80 cases with TN treated by GKS and 60 cases with TN treated by RT in our department in the past 5 years were performed.Treatment outcomes were assessed through patient self-reports of pain control and medication usage during follow-up visits.Results All patients completed questionnaires for a median follow-up of 22 months(range:6~38 months).The GKS cures a year efficient rate is 97.5%;two years efficient rate is 96.3%;but the RT cures a year efficient rate is 96.8%;two years efficient rate is 75%.GKS was better than RT.The common complications included numdness,absence of corneal reflex and master weakness,the RT is more than the GKS(P<0.01).Conclusion GKS is a minimally invasive technique for the treatment of primary trigeminal neuralgia with few complications.RT is an effective and promising method in the treatment of TN.Practitioner should pay attention to pain relief and reduce complications.Further studying is necessary to adequate the durability of response.
