摘要
目的评估肛垫保留术治疗痔的可行性、有效性及肛管功能恢复的完整性。方法对150例痔患者手术治疗时保留肛垫,肛管移行上皮(ATZ)收紧提吊断裂松驰的Treitz肌,仅对引起痔症状下移的痔组织切除。对术后疼痛、出血、住院时间、伤口愈合、并发症、肛管功能恢复情况进行临床观察及远期疗效评估。结果术后疼痛24小时内需注射镇痛剂的29例,占19%,疼痛不明显或轻度疼痛无需镇痛治疗的121例,占81%。24小时后全组病人无作镇痛治疗。伤口愈合时间2～6周。住院天数3～14天,平均7.9天。无大出血并发症发生,无控便及排便障碍。随访2年以上,全组病例无肛门狭窄、控便及排便功能障碍,疗效评级:优132例,占88%;良17例,占11.3%,差1例0.7%。结论肛垫保留术治疗痔的术式是一种简单,有效,可靠,远期疗效优良,肛管功能恢复完全,可替代传统痔切除术的术式。
Objective To evaluate the effect of anal cushions preserved hemorrhoidectomy for hemorrhoidal treatment.Methods 150 patients with hemorrhoid were treated by anal cushions preserved hemorrhoidectomy and postoperative pain,hemorrhage,the time of in hospital,the time of wound healing,complications and anal function were observed. Results Among 150 cases,29 cases needed analgesic for injection in 24 hours after operation.The time of wound healing was 2～6 weeks.The time of in hospital was 3～14 days.No severe bleeding,anal stenosis,disturbed defication and incontinence were found. Therapeutic evaluation:excellent was in 132 cases(88%)good was 17 cases(11.3%),and poor was 1 case(0.7%). Conclusion This new techniques is simple,safe,effective,rapid recovery,minimized postoperative pain and it can replace the traditional Hemorrhoidectomy.
