摘要
目的探讨梗阻性结肠癌的手术时机、处理方法与吻合口瘘的关系。方法对我院收治的62例梗阻性结肠癌作回顾性分析,根据其主要临床表现为肠梗阻,作充分的术前准备,术中根据具体情况来选择相应的术式:Ⅰ期切除;先行造瘘、后行Ⅱ期切除或单纯造瘘结合术后化疗。结果I期切除吻合38例中有1例发生吻合口瘘(2.6%),Ⅱ期吻合术11例中没有一例发生吻合口瘘。结论严格掌握手术适应症,合理选择手术方式,术中有效肠道减压,清洁肠腔,正确的吻合技术及作好围手术期的处理,才能有效的预防吻合口瘘的发生。
Objective To study the operative timing and managed method correlated with anastomotic leakage. Methods From July1990to Augest2001 ,62cases in the treatment of the obstructive colonic cancer were analyzed retrospectively.According to the patient's status at that time ,the different operative methods were selected. Results Among38cases of primary resection and anastomosis ,anastomotic leakage occurred in one case ( 2.6% ) .In the11cases of second anastomosis ,no anastomotic leakgge was found. Conclusion For preventing anastomotic leakage effectively ,it is very important to strict controlled operation indication ,reasonable selection of operative manner ,effective intestinal decompressian in the intraoperation ,cleaning intestinal cavity ,correct anastomotic techneque and pay attention to intestinal circulation ,anastomosis-tension free ,periopeiative treatment ,ect.
