摘要
目的探讨扩散加权成像(DWI)诊断缺氧缺血性脑病(HIE)的最佳时间窗。方法对临床确诊的81例HIE患儿(年龄12h至38 d,平均11.49 d;男63例,女18例)行常规T1WI、T2WI和DWI检查(b=1000 s/mm2),分为5组[Ⅰ组(≤4d)20例、Ⅱ组(5~7d)12例、Ⅲ组(8~14d)23例、Ⅳ组(15~21d)18例和Ⅴ组(≥22d)8例]。在DWI、T1WI上观察比较皮层及皮层下白质、深部白质、基底节及丘脑、脑室及脑外间隙等部位病灶数目、范围、清晰度,并进行独立样本t检验。结果在显示病灶数目、大小和清晰度方面:I组中DWI均明显优于T1WI(P值均<0.01);Ⅱ、Ⅲ组中DWI和T1WI均无明显统计学差异(P值均>0.05);Ⅳ、Ⅴ组中DWI均不及T1WI(P值均<0.05)。结论DWI在HIE不同时期的诊断价值不同,4天内是DWI诊断HIE的最佳时间窗。
