摘要
目的探讨背驮式肝移植手术的术式改良。方法结合文献介绍之术式,对几种改良术式进行评价。结果改良背驮式肝移植术式在病肝切除时间、肝流出道重建时间、无肝期及术中出血量等指标上,优于传统背驮式肝移植术;4种改良背驮式肝移植术式中,3种不保留肝静脉干的吻合术式优于保留肝静脉干的吻合术式。结论供受体下腔静脉吻合的改良背驮式肝移植应为背驮式肝移植的首选术式。
Objective To discuss the innovation of operation methods for piggyback liver transplantation(PBLT).Methods According to our and others experience in PBLT,several innovation of operative methods were compared and evaluated.Results Compared to standard PBLT(SPBLT),ameliorative PBLT(APBLT) reduced the duration of whole hepatectomy,hepatic outflow tract reconstruction,and anhepatic period,as well as reduced intraoperative bleeding.Among the four kinds of APBLTs(three of them with hepatic vein,the other one without),anastomosis without hepatic vein was better than others.Conclusion Among the APBLTs,APBLT with the inferior vena cava anastomosis is preferred to others.
