过度通气对动物颅内高压时脑组织氧及二氧化碳分压的影响 被引量:3

摘要 目的:研究过度通气对猪急性脑内血肿颅内高压状态下脑组织氧分压(PtiO2)及二氧化碳分压(PtiCO2)、pH值和颅内压(ICP)的影响及其意义.方法:利用微泵注射自体血法建立猪急性脑内血肿模型,注入自体血并待颅内压稳定在较高水平(4.0~5.33 kPa)后,给予过度通气15 min,使PaCO2<4.0 kPa,连续监测平均动脉压(MAP)、ICP、脑灌注压(CPP)、PtiO2、PtiCO2、pH和HCO3-的变化及动脉血气变化.结果:脑内血肿组动物过度通气后,ICP、PtiCO2显著下降,CPP、pH值明显上升,而PtiO2却急剧下降至缺血水平.结论:过度通气可显著地降低ICP,但也显著地降低脑PtiO2,因此颅脑外伤后早期应避免使用过度通气.
