一种血液透析内瘘的新术式—动静脉2针套接法 被引量:4

摘要 目的 总结并报告一种简捷、安全的永久性血液透析直接内瘘成形术的新术式。方法 慢性肾能衰竭尿毒症期(CRF)行血液透析(HD)治疗的患者90例分3组:A组30例采用2针套接内瘘新术式;B组:30例采用钛制轮钉内瘘术;C组:30例采用端端缝合内瘘术。3组内瘘均于4周后使用;并比较3组血液透析时血流量及内瘘血栓和血管瘤的发生率以及3组内瘘手术中血管连接时间。结果 2针套接成熟时间及透析血流量均达标准;其血管连接时间明显较另两组短(15.3±2.4)min,P<0.01;其血栓发生率较另两组低(6.7%,P<0.01)且无血管瘤发生。结论 动静脉2针套接法是一种简捷、安全、血管并发症少且血管连接处无异物和狭窄的新术式。 Objective We study and report a new operation method of internal arteriovenous fistula, and it is a simple, fast and safe method. Methods 90 CRF patients were treated with hemodialysis. We divided the patients into three groups. There were 30 patients in every group. In A group, the internal arteriovenous fistula were performed operation with new method. In B group, the internal arteriovenous fistula conneted with titamiu ring nail. In C group, the artery extremity and vein extremity were connected with stitch method. We used internal arteriovenous fistula for hemodailysis four weeks after operation. The blood flux of them, the occuring rate of thrombus, angiomatous and the time of operation were observed. Rasults The internal arteriovenous fistula mature time and the blood flux were sained in three groups. In A group, the operation time was shorter than other group ( P < 0.01), the occuring rate of thrombus is lower than other groups (P < 0.01). There were not any angiomatous in A group. Conclusion The new skill method is simple, fast and safe. There were not any eyewinke and stenosis at arteriovenous connecting.
