摘要 目的探讨凋亡相关基因Bcl-2和Bax在增生性瘢痕(HS)中的表达水平和分布特征及其与瘢痕增生和退化关系。方法检测HS中Bcl-2和Bax的表达。结果正常皮肤不表达Bcl-2,在HS中Bcl-2和Bax的阳性率为100%。浅层Bcl-2阳性细胞率高达64%~78%,显著高于其他时间点标本和同一标本的中层和深层(P< 0.01),仅成纤维细胞表达阳性;Bax在瘢痕增生的全过程和各层组织均有较高的阳性细胞率(52%~69%)和广泛的细胞分布,显著高于正常皮肤(P< 0.01)。结论深度烧伤创面愈合后,浅层成纤维细胞过度表达Bcl-2可能与创伤修复失控和HS形成有关,而广泛的Bax蛋白的过度表达可能具有抵抗Bcl-2活性作用,加速细胞的生理性凋亡和瘢痕成熟。 :Objective To study the expression and histological distributive characteration of apoptosis-related gene Bcl-2 and Bax in hypertrophic scars (HS) and the relationship to hypertrophia and degeneration of scars. Method Detect the expression of Bcl-2 gene and Bax gene in HS.Result There was no expression of Bcl-2 in normal skin ,and the positive ratio of Bcl-2 and Bax were 100%in HS.The positive ratio of Bcl-2 was 64%~78%in superficial layer, and significantly higher than other specimen groups of different times, and higher than the middle layer and the deep layer of the same specimen group(P< 0.01).The immunoreactive positive cells were fibroblasts. The positive ratio of Bax was significantly higher than the normal skin (P< 0.01)and in the whole course of hypertrophic scars(52%~69%), and the immunoreactive positive cells of Bax were distributed extensively.Conclusion The overexpression of Bcl-2 in fibroblast might be related to out of control of wound healing and the formation of HS after the wound healing of deeper-partial burns.The extensive overexpression of Bax protein might be the active resistance effect of Bcl-2 accelerating physiological apoptosis of cells and maturation of scars.
