摘要
目的探讨有冠状动脉慢血流现象的患者阵发性心房颤动(paroxysmal atrial fibrillation PAF)与冠脉慢血流的关系。方法选择冠状动脉造影正常但存在冠脉慢血流的患者66例为I组,男38例,女28例。冠脉造影正常无冠脉慢血流的患者88例为II组,男51例,女37例。冠状动脉血流情况按照校正的TIMI血流分级方法评估。所有患者在住院后随访1年,行动态心电图检查有无PAF。P波离散度(Pd)从体表12导联心电图测得。计算PAF在各组的发病率。结果两组年龄、性别、高血压病、心力衰竭、糖尿病、高脂血症、抽烟史等均无明显统计学差异。I组患者Pd与II组比较明显延长(39.3±12.6)vs21.5±10.8milliseconds,P<0.01).,PAF发病率明显比II组升高(18.2%vs7.9%,P<0.01)结论在有冠状动脉慢血流现象的患者中患者P波离散度较对照组明显延长,阵发性心房颤动发病率升高。
Objective To investigate the paroxysmal atrial fibrillation (PAF)in patients with coronary slow flow (CSF) phenomenon,and to evaluate the Relationship between PAF and coronary slow flow.MethodsStudy population included 66 patients with angiographically proven normal coronary arteries and slow coronary flow in all 3 coronary vessels (group I, 38men; mean age, 63.9±10.3years) and 88 subjects with angiographically proven normal coronary arteries without associated slow coronary flow (group II, 51 men, mean age, 62.7±11.4years).Conclusion Flow rates of all patients and control subjects were documented by Thrombolysis In Myocardial Infarction (TIMI) frame count. The mean TIMI frame count for each patient and control subject was calculated by adding the TIMI frame counts for each major epicardial coronary artery and then dividing the obtained value into 3. The P wave dispersion (PD) were measured from the 12-lead surface electrocardiogram The PAF were checked from Dynamic Electrocardiogram.Results There was no statistically significant difference between the 2 groups with respect to age, sex, hypertension, congestive heart failure , diabetes mellitus, hyperlipidemia, and cigarette smoking (P>0.05). Pd of patients with CSF were found to be significantly higher than those of control subjects (39.3± 12.6)vs(21.5 ±10.8)milliseconds,P<0.01). The incidence of PAF were found to be higher in patients with CSF than in controls.( 18.2%vs7.9% )Conclusion: P-wave dispersion were found to be greater in patients with CSF than in controls.The incidence of PAF were found to be higher in patients with CSF than in controls.
