质子磁共振波谱技术研究帕金森病壳核神经代谢改变

Biochemical Changes in Substantia Putamen of Patients with Parkinson Disease:Observation by Proton Magnetic Resonance Spectroscopy

摘要 目的利用质子磁共振波谱(1H-MRS)技术研究帕金森病(PD)患者壳核的代谢变化及意义。方法选择15例PD患者作为PD组,依据Hoehn和Yahr分期,将PD组分成早期PD组(H/Y分级<Ⅱ级)和晚期PD组(H/Y分级≥Ⅱ级),另选择13例健康志愿者作为对照组。3组分别予以双侧壳核1H-MRS检查,测定双侧壳核N-乙酰天门冬氨酸/肌酸(NAA/Cr)、N-乙酰天门冬氨酸/胆碱复合物(NAA/Cho)和胆碱复合物/肌酸(Cho/Cr)值。结果早期PD组患者NAA/Cr、NAA/Cho、Cho/Cr值分别为(1.3±0.3)、(1.16±0.30)和(1.13±0.27),晚期PD组分别为(1.2±0.5)、(1.14±0.44)、(1.16±0.25),对照组分别为(2.3±0.4)、(1.64±0.28)、(1.43±0.32)。早期PD组和晚期PD组NAA/Cr、NAA/Cho值与对照组比较差异有统计学意义(P<0.05)。结论1H-MRS技术可以无创性地为PD患者双侧壳核内神经代谢改变提供有价值的信息。 Objective To evalutate the metabolism of the putamen by proton magnetic resonance spectroscopy(1H-MRS) in patients with Parkinson disease(PD).Methods Fifteen PD patients were divided into 2 groups according to Hoehn-Yahr stage.Thirteen healthy volunteers were enrolled as control in this study.MRS was used to determine N-acetylaspartate/chline(NAA/Cho),N-acetylaspartate/creatine(NAA/Cr) and the ratio of Cho/Cr in putamen.Results In PD patients,the ratios of NAA/Cr and NAA/Cho in putamen reduced significantly and the ratio of Cho/Cr increased〔(1.3±0.3),(1.16±0.30),(1.13±0.27),respectively,in early stage,and(1.2±0.5),(1.14±0.44),(1.16±0.25),respectively,in late stage〕 as compared with control〔(2.3±0.4),(1.64±0.28),(1.43±0.32),respectively〕,and there was not significant difference in ratio of Cho/Cr between control group and PD patients(P<0.05).Conclusion 1H-MRS is a noninvasive technique providing useful informations on metabolic changes in the bilateral putamen of PD patiennts.
