A 56-year-old man with no significant medical history received implant surgery in the site of his left mandibular molar in2001,08,30. Examination:Soft tissue of maxilofacial region was symmetrical with no swell. Moderate periodontitis had been treated.#37 had been lost for over two years,the lip of alveolar was absorbed moderately.#36 showed moderate-caries on occlusal surface,no Obvious lossement,with dark red of gingiva. X-ray showed the bone of #37 available。He received the implant surgery under local anesthesia. Four months later,the implant showed no loosement and no uncomfortable and received the secondary surgery and restoration treatment. 14 months later,the implant was loose in class I,and X-ray showed low density around apical of #36 and #37. The gingival at #37 was not swell.We removed the implant with white bone particles. We concluded that periapical periodonititis of adjacent teeth could affect the success of the implant.
