摘要
目的 探讨夜戴和日戴 Ortho- K接触镜 1a期间降低近视的疗效及问题点。方法 ≤ 5.0 0 D的 4 5名近视患者为 组 ,采用过夜戴镜方式。 >5.0 0 D的 3 7名近视患者为 组 ,采用日间戴镜方式。对 2组戴镜 1a期间裸眼视力的提高 ,近视度的降低 ,角膜曲率、角膜地形图的改变 ,近视降低度与角膜曲率变化的关系 ,镜片更换的时间及原因进行分析研究。结果 组戴镜后裸眼视力提高至 1.0~ 1.5的比率 ,1个月为 64 .1% ,3个月为 65.2 % ,6个月为56.2 % ,1a后 3 3 .7% .戴镜后 2组近视度数均显著降低 ,3个月后最为明显 ,分别降低 3 .4 6D± 0 .93 D和 3 .96D± 1.2 4 D,1a左右出现回退。角膜曲率半径平均值的增加 ,2组在戴镜 6个月后最显著 ,1a左右出现回退。角膜地形图 Sim K值的变化 , 组戴镜 3个月后 , 组戴镜 6个月后最显著。 组戴镜 3个月后 ,近视度降低值与角膜地形图 FK变化值有显著性相关 (r=0 .559,P <0 .0 1)。镜片调换时间多数在 1个月之内 ,因为配适不佳或疗效欠佳 ;在戴镜后 1a左右 ,因为镜片老化、变形、沉淀物而更换新片。 2组戴镜期间均无明显角、结膜合并症出现。结论 科学规范使用夜戴和日戴 Ortho- K接触镜均可在短时间内迅速降低近视度 ,持续 1a左右出现回退。镜片寿命为 1a左右 。
Objective To evaluate the effect of Ortho-K contact lens overnight wear and daily wear on the myopia reduction within one year.Methods Forty-five subjects had 5.00D or less than 5.00D of myopia (group I) with overnight wear. Thirty-seven subjects had over than 5.00D of myopia (group II) with daily wear. We observed the changes of unaided visual acuity(UVA), myopia diopters, corneal curvature, topography and the relationship between the change of refractive error and the change of cornea K readings. The time and causes of lens exchange were analyzed also.Results The results of UVA which improved to 1.0~1.5 after orthokeratology in group I were 64.1% (1month),65.2%(3 months), 56.2%(6 months) and 33.7%(1 year). Both groups showed significant reduction of myopia after Ortho-K contact lens wearing. The maximum reduction occurred at 3 months, 3.46D±0 93D and 3 96D±1.24D respectively, the reversion was found at about one year. The maximum flat changes of average cornea K readings at six months and regression at about one year. The topography K readings changed more obviously at 3 months (group I ) and at 6 months (group II). There was a significantly relationship between myopia reduction and FK change of topography in group II at three months ( r =0.559, P <0.01). The improper fitting or poor effect often cause the lens exchange within one month and a total lens exchange occurred about one year because of lens aging, distortion and deposits. No remarkable corneal and conjunctival complications were found during the period of treatment. Conclusion With the overnight wear and daily wear of the Ortho-K contact lens can reasonabally reduce myopia repidly in short time, but regression occurs at about one year. The life of Ortho-K contact lens should be considered as one year. It can improve and continue effectiveness when the lenses are changed in time. The daily wear which is easy to keep,lits well,obtains good vision results with regular changes and has less influence on the cornea and conjunctiva, more safer tha
