摘要
背景与目的:本研究探讨经阴道彩色多普勒超声(trasvaginalcolorDopplerflowimaging,TV-CDFI)与多项血清肿瘤标志物联合应用对卵巢肿瘤的诊断价值。方法:对74例盆腔肿块患者(其中卵巢良性肿瘤30例、恶性肿瘤44例)进行TV-CDFI检查,同时检测7项血清肿瘤标志物进行诊断,并将诊断结果与病理诊断结果进行对照。结果:单独应用TV-CDFI、血清肿瘤标志物检测对卵巢肿瘤的诊断符合率分别为90.5%、59.5%。将TV-CDFI检查与血清肿瘤标志物检测结合,诊断符合率可达98.6%,其中恶性肿瘤诊断符合率为97.7%。结论:TV-CDFI可对多数卵巢肿瘤作出诊断和鉴别诊断,并可检出部分早期卵巢癌;与多项血清肿瘤标志物检测联合应用,能够进一步改善诊断的敏感性和特异性,提高早期诊断和鉴别诊断水平。
BACKGROUND &OBJECTIVE: To investigate the value of trasvaginal co lo r Doppler flow imaging(TV-CDFI ) in combination with multiple serum tumor marke rs in the diagnosis of ovarian tumors. METHODS: A total of 74 patients with pelv ic masses ( 30 cases with benign tumors, 44 cases with malignant tumors) were ex amined by TV-CDFI and the level of 7 serum tumor markers was also measured at s ame time. The above results were compared with the pathologic findings. RESULTS: The diagnostic accuracy for the TV-CDFI only, and 7 serum tumor markers measur ed were 90.5%and 59.5%respectively. If TV-CDFI combined with multiple serum t umor marker measurements, the diagnostic accuracy of 98.6%was achieved, whereby it was 97.7%for malignant tumors. CONCLUSION: TV-CDFI allows the diagnosis an d differential diagnosis of the majority of ovarian tumors, particularly in part of ovarian carcinoma at early stage. Combined with multiple serum tumor marker measurements, this method can improve diagnostic sensitivity and specificity, he nce it facilitates the early diagnosis and differential diagnosis.
