摘要
目的观察真核细胞翻译起始因子4E(eIF4E)及磷酸化eIF4E结合蛋白1(p-4EBP1)在甲状腺乳头状癌组织中的表达,并探讨其与甲状腺乳头状癌的侵袭、转移等临床病理特征间的关系。方法采用免疫组化EnVinsion法检测66例乳头状癌(经典型乳头状癌37例,滤泡型乳头状癌29例)、20例结节性甲状腺肿伴乳头状增生及上述病例的肿瘤旁正常甲状腺组织中eIF4E与p-4EBP1的表达。结果在肿瘤旁正常甲状腺组织、甲状腺肿伴乳头状增生及乳头状癌中,eIF4E的阳性率分别为11.6%(10/86)、50.0%(10/20)和84.8%(56/66)(P<0.01);p-4EBP1的阳性率分别为8.1%(7/86)、40.0%(8/20)和66.7%(44/66)(P<0.01),并且p-4EBP1在滤泡型乳头状癌中的表达显著高于其在经典型乳头状癌中的表达(P<0.01)。eIF4E在有腺外侵袭和淋巴结转移的病例中的阳性率分别为96.7%(29/30)和100.0%(29/29),分别高于无腺外侵袭和无淋巴结转移的病例,后者阳性率分别为75.0%(27/36)和73.0%(27/37)(P<0.05)。而p-4EBP1在这两组病例中的表达差异则无显著性意义(P>0.05)。结论和正常甲状腺及甲状腺良性病变相比,eIF4E和p-4EBP1在乳头状癌中存在过表达;eIF4E的过表达与乳头状癌的侵袭与转移关系密切,在乳头状癌的进展中可能发挥了重要作用。
Objective To evaluate the expression of eukaryotic initiation factor 4E(eIF4E) and phosphorylated eIF4E binding protein 1(p-4EBP1) in papillary thyroid carcinoma(PTC) and its association with the clinicopathologic properties such as invasion and metastasis of PTC.Methods Immunohistochemistry was performed in a series of formalin-fixed,paraffin-embedded tissues including 66 cases of PTC(37 cases of conventional PTC and 29 cases of follicular variant of PTC),20 cases of nodular goiter with papillary hyperplasia(NGPH) and corresponding normal thyroid tissues(NTT) adjacent to the tumors.Results The positive rates of eIF4E in NTT,NGPH and PTC were 11.6%(10/86),50.0%(10/20) and 84.8%(56/66) respectively(P<0.01),and those of p-4EBP1 were 8.1%(7/86),40.0%(8/20) and 66.7%(44/66) respectively.Furthermore,the positive rate of p-4EBP1 in folluliar variant of PTC was significantly higher than that in conventional PTC(P<(0.01).) The eIF4E expression level was significantly higher in both extrathyroidal invasion group and lymph node metastasis group than that in intrathyroidal invasion group and non-lymph node metastasis group(P<0.05).The expression of p-4EBP1 had no significant difference among these groups(P>0.05).Conclusion eIF4E and p-4EBP1 were overexpressed in PTC compared with normal tissue and papillary hyperplasia of thyroid.eIF4E may play an important role in the progression of papillary thyroid carcinoma.
