后路AF系统固定加植骨治疗胸腰椎爆裂骨折的疗效观察 被引量:3

摘要 目的探讨后路AF系统固定加植骨治疗胸腰椎爆裂骨折的疗效。方法39例病人行后路AF系统固定并加植骨治疗,其中经椎管椎体内植骨6例,椎体内植骨并椎间融合7例,椎间植骨融合4例,椎板关节突植骨融合22例。观察伤椎椎体前后缘高度,Cobb氏角及神经功能变。结果伤椎前﹑后缘高度,Cobb氏角术前分别为51.9±7.2,67.7±10.1,22.1±6.6,术后分别为94.0±9.8,97.3±8.1,3.1±1.7,术后神经功能较术前有较大的恢复。结论后路AF系统内固定治疗胸腰椎爆裂骨折,固定可靠,并具良好的撑开作用,不仅恢复了椎体高度,重建脊柱的生理曲度,并为神经功能的恢复创造了条件。而植骨则有利于建立脊柱自身的骨性稳定。 Objective To study the efficacy of using AF system through posterior approach and gfafting bone in the treatment of thoracolumber burst fractures.Methods 39cases of thoroacolumber burst fractures were fixed by AF system,and 6cases with bone grafted in vertebral bodyfrom spine cannel,7cases with bone graftied in vertebral body and fused through interspinous,4cases fused through interspinous,22cases fused through articular process of vertebral lamina.The change of the height of the anterior and posterior vertebrall body,the cobb's angle and the nerve function wre observed.The period of follow up for all partients was ranging from six months to two years,being fifteen months on the average.Results The anterior and posterior vertebral body height and Cobb's angle preoperation were 51.9±7.2,67.7±10.1,22.1±6.6,but they wre 94.0±9.8,97.3±8.1,3.1±1.7 pos-toperation.The never function was showed much better than befor as well.Conclusion Using AF system through posterior approach might be a good internal fixation system in spine fracture,especially the fracture blocks entered into the vertebral canal were reduced,height of vertebral body was raised and chance of recovery of never function was provided.Grfating bone can help spine to build itself stability.
