腰椎后柱“骨-腱-骨”结构的完整性对邻近节段生物力学的影响 被引量:10

摘要 目的 探讨腰椎后柱'骨-腱-骨'结构的完整性对邻近节段生物力学的影响.方法 取1月龄小牛腰椎标本5具,于万能生物力学试验机上模拟人体不同工况,对L3~4节段进行力学测定;随后进行各种模拟手术,于L4~5安装内固定并逐级切除后柱'骨-腱-骨'结构,依次测定经单侧椎板开窗髓核摘除(UTD组)、单侧椎板切除+双侧椎弓根钉和单枚融合器(BFC组)、双侧椎板及部分'骨-腱-骨'结构切除+双侧椎弓根钉和单枚融合器(BFC+PBTB组)、双侧椎板及全部'骨-腱-骨'结构切除+双侧椎弓根钉和单枚融合器(BFC+TBTB组)处理后L3~4节段力学变化情况.结果 行UTD后,邻近节段小关节应变、应力及椎间盘应力在前屈和侧屈时均有所增加,但差异无统计学意义(P>0.05).行BFC后,在不同工况下椎间盘上的应力明显异常(P<0.05),前屈时小关节的应变和应力增加明显(P<0.05).行BFC+PBTB/TBTB后,在不同工况下小关节应变、应力及椎间盘上应力的增加差异均具有统计学意义(P<0.05).腰椎轴向刚度在BFC组增加达到218.33 N/mm(P<0.05),但在BFC+PBTB/TBTB组均下降,其中后两者分别下降17%、25%,与正常标本相比差异有统计学意义(P<0.05).结论 腰椎融合内固定术中,后柱'骨-腱-骨'结构完整性的破坏有增加术后上位节段退变的趋势.
