摘要
目的研究宫颈癌筛查过程中患者行阴道镜/细胞学检查前后的心理变化,为采取针对性措施提供依据。方法随机抽取妇科门诊通过妇科检查发现宫颈病变的患者200名,按就诊比例随机分为两组,一组行阴道镜检查,一组行细胞学检查(薄层液基细胞学检测技术),以筛查宫颈癌。记录、观察两组患者筛查宫颈癌前后的血压、脉搏,并使用焦虑自评量表测评患者的心理状态。结果通过阴道镜筛查宫颈病变后患者的焦虑程度较强烈,阴道镜检查组与细胞学检查组进行宫颈癌筛查后焦虑自评量表值间的比较,差异有显著性意义(t=2.36,P<0.05);阴道镜组患者检查后的收缩压、舒张压和脉搏变化的差异均有显著性意义(t=2.65、2.47、2.25,均P<0.05)。结论在宫颈癌的筛查过程中,应注重对患者进行心理调整,提高患者对宫颈病变的认识和重视,有助于配合宫颈癌的筛查,提高妇女生活质量。
Objective To investigate psychological changes of patients after being diagnosed as cervical carcinoma by colposcopy and cytological examination so as to provide a basis for aimed interventions.Methods 200 outpatients who had been diagnosed by gynecological examinations as cervical diseases were divided into two groups at random(according to visit order): the patients in the first group underwent colposcopy for cervical carcinoma screening,and the patients in the second group underwent cytological examination for cervical carcinoma screening.Blood pressure and pulse of the patients in the two groups before and after cervical carcinoma screening were recorded and compared.Self-Rating Anxiety Scale was used to test and analyze psychological changes of the patients.Results The anxiety degree of those patients after being diagnosed as cervical carcinoma by colposcopy was more severe.In Self-Rating Anxiety Scale score after cervical carcinoma screening,there was significant difference between the two groups(t=2.36,P<0.05).The changes in systolic pressure,diastolic pressure and heart rate of patients in the first group were significant before and after being diagnosed as cervical carcinoma by colposcopy(t=2.65,2.47,2.25,respectively,all P<0.05).Conclusion In course of diagnosis and treatment of cervical carcinoma,psychological state of the patients should be concerned.Moreover,their knowledge about cervical diseases should be educated,which is contributive to co-operation of the patients in cervical carcinoma screening to improve their quality of life.
