摘要
目的基于ICU护士持续性肾脏替代治疗(CRRT)培训知识需求,构建ICU护士CRRT规范化培训知识体系。方法采用便利抽样法于2021年6月至2022年6月选取3家三级甲等医院ICU护士开展CRRT培训知识需求调查;结合调查结果及文献回顾确定原始培训知识,应用德尔菲法对不同地区10名专家进行2轮函询,形成ICU护士CRRT规范化培训知识体系。结果132名ICU护士参与调查,81.82%的ICU护士独立承担CRRT操作,75.76%经常开展CRRT操作;78.79%能够定期接受CRRT培训,78.79%具有非常高或比较高的CRRT培训需求,且需求知识主要集中在CRRT报警,相关并发症,突发事件应急预案,CRRT联合体外膜肺氧合(ECMO),接ECMO不同位置,CRRT有无抗凝技术及监测,CRRT的闭路循环、上机及下机操作流程等;知识体系构建阶段,2轮专家函询问卷有效回收率均为100.00%,专家权威系数分别为0.865、0.875,肯德尔协调系数分别为0.287、0.331(P<0.001);构建的ICU护士CRRT规范化培训知识体系包含一级指标5个、二级指标45个,各级指标重要性评分均数基本在4.30~5.00分。结论基于需求调查及德尔菲法构建的培训知识体系具有一定的科学性及合理性,专家对条目重要性较为认可,可为ICU护士CRRT培训提供参考。
Objective To establish a comprehensive standardized training knowledge framework continuous renal replacement therapy(CRRT)tailored to ICU nurses'needs,derived from an investigation into their specific knowledge requirements.Methods A survey was conducted among ICU nurses from three tertiary hospitals between June 2021 and June 2022,utilizing convenient sampling.The original training knowledge was determined by combining the survey results and literature review.The final knowledge system index of CRRT standardized training for ICU nurses was determined by two rounds letter consultation of 10 experts from different regions.Results A total of 132 nurses participated in the survey.81.82%of ICU nurses operated CRRT independently,with 75.76%regularly performing CRRT procedures.Moreover,78.79%received consistent CRRT training,expressing substantial demand for knowledge enhancement.Their specific needs centered on critical areas like CRRT alarms,related complications,emergency plan,the integration of CRRT with ECMO,different connection methods,anticoagulation techniques,closed-circle methodologies,and operational protocols.In the construction phase of the knowledge system,the first and second rounds of questionnaires achieved a response rate of 100.00%,supported by authority coefficients of 0.865 and 0.875,respectively(P<0.001).Additionally,Kendall's W values were 0.287 and 0.331(P<0.001)for the first and second rounds,respectively.This rigorous process culminated in a final system comprising 5 primary indicators,alongside 45 secondary indicators.Notably,all indices demonstrated high importance scores ranging from 4.30 to 5.00.Conclusion The knowledge system,meticulously crafted through demand-based surveys and the Delphi method,stands as a scientifically grounded and logical framework.It stands poised to serve as a valuable reference point for shaping effective CRRT training programs tailored specifically for ICU nurses.
