摘要
目的探讨老年腹股沟疝患者术后出院准备度量表(RHDS)评分现况的影响因素及干预策略。方法回顾性分析2020年8月至2022年6月淮南新华医院收治的90例老年腹股沟疝术后患者的临床资料。比较不同情况老年腹股沟疝患者术后RHDS评分,将其中有统计学意义的因素纳入多因素分析,并予以多元线性回归分析法分析老年腹股沟疝患者术后RHDS评分现况的影响因素。结果年龄>75岁、高中及以下、居住地为农村、斜疝、开放手术、自己照顾、独居、合并慢性疾病、感知自理能力下降、术后局部水肿、术后尿潴留、术后恶心呕吐、重度疼痛的老年腹股沟疝患者术后RHDS评分较低(P<0.05)。多元线性回归分析结果显示:年龄>75岁、居住地为农村、斜疝、开放手术、感知自理能力下降、术后局部水肿、术后尿潴留、术后恶心呕吐、重度疼痛是老年腹股沟疝患者术后RHDS评分现况的独立危险因素。结论年龄>75岁、居住地为农村、斜疝、开放手术、感知自理能力下降、术后局部水肿、术后尿潴留、术后恶心呕吐、重度疼痛是老年腹股沟疝患者术后RHDS评分现况的独立危险因素,临床可据此给予针对性的治疗及干预措施,以提高患者出院准备度。
Objective To explore the influencing factors and intervention strategies of scores of discharge Readiness Scale(RHDS)in elderly patients after inguinal hernia surgery.Methods The clinical data of 90 elderly patients after inguinal hernia surgery who were admitted to Huainan Xinhua Hospital from August 2020 to June 2022 were analyzed retrospectively.The scores of RHDS of elderly inguinal hernia patients with different conditions were compared,the factors with statistical significance were included in the multivariate analysis,and the influencing factors of the scores of RHDS in elderly patients after inguinal hernia surgery were analyzed by multiple linear regression analysis.Results The scores of RHDS were lower in elderly patients who were aged>75 years old,high school and below,living in rural areas,indirect hernia,open surgery,self-care,living alone,with chronic diseases,had decreased ability to perceive self-care,postoperative local edema,urinary retention,nausea and vomiting,and severe pain(P<0.05).Multiple linear regression analysis showed that the age>75 years old,residence in rural areas,indirect hernia,open surgery,decreased perceived self-care ability,postoperative local edema,urinary retention,nausea and vomiting,and severe pain were independent risk factors for the score of RHDS in elderly patients after inguinal hernia surgery.Conclusion Age>75 years old,living in rural areas,indirect hernia,open surgery,decreased self-care ability,postoperative local edema,urinary retention,nausea,vomiting and severe pain were independent risk factors for the score of RHDS in elderly patients after inguinal hernia surgery.Targeted clinical treatment and intervention measures could be given accordingly to improve patient's readiness for discharge.
